摘  要:目的探究兰州市气态污染物[SO_(2)、NO_(2)、臭氧8小时平均浓度(O_(3)8h)]与痤疮门诊人次的相关性。方法收集2013年1月至2017年12月痤疮门诊人次的临床资料和相应的SO_(2)、NO_(2)、O_(3)8h的浓度。在控制长期趋势与星期几效应等混杂因素的基础上,运用分布滞后非线型模型研究空气中SO_(2)、NO_(2)、O_(3)8h对痤疮门诊人次的滞后效应和累积效应;按照性别、年龄不同人群建立模型。结果平均每日就诊痤疮患者(34.94±15.27)例;SO_(2)、NO_(2)、O_(3)8h平均浓度分别为24.09、43.77、82.94μg/m^(3),经Spearman相关分析后显示痤疮门诊人次与NO_(2)呈正相关,与O_(3)8h呈负相关;SO_(2)与O_(3)8h呈负相关;NO_(2)与SO_(2)、O_(3)8h呈正相关。SO_(2)、NO_(2)浓度每增加10μg/m^(3),门诊人次分别增加2.61%(95%CI=0.98%~4.27%)、1.71%(95%CI=0.54%~2.89%);O_(3)8h浓度每增加10μg/m^(3),门诊人次增加-0.62%(95%CI=-1.12%~-0.11%)。多污染物模型分析提示各空气污染物对痤疮门诊人次仍有影响。结论兰州地区SO_(2)、NO_(2)与痤疮门诊人次呈正相关,O_(3)8h与痤疮门诊人次呈负相关,各性别、年龄人群痤疮门诊人次对SO_(2)、NO_(2)、O_(3)8h浓度变化的敏感程度各不相同。Objective To determine the relationship between air pollutants[SO_(2),NO_(2),8 hours average concentration of ozone(O_(3)8h)]and outpatient visits for acne in Lanzhou,China.Methods Data of daily outpatient visits for acne and concentrations of air pollutants(SO_(2),NO_(2),and O_(3)8h)from January 2013 to December 2017 were collected.With the control of long-term trend and day-of-week effect,a distributed lag nonlinear model was used to analyze the lag and cumulative effects of air pollutants on the outpatient visits.The model was established based on the different groups of gender and age.Results During the study period,the average daily outpatient visits were 34.94±15.27,and the average concentrations of SO_(2),NO_(2),and O_(3)8h were 24.09,43.77,and 82.94μg/m^(3),respectively.The Spearman’s correlation analysis showed that the daily outpatient visits for acne were positively correlated with NO_(2)concentration while negatively correlated with O_(3)8h concentration;SO_(2)concentration was negatively correlated with O_(3)8h concentration;NO_(2)concentration was positively correlated with SO_(2)and O_(3)8h concentrations.A 10μg/m^(3)increase in daily concentrations of NO_(2)and SO_(2)was respectively associated with 2.61%(95%CI=0.98%-4.27%)and 1.71%(95%CI=0.54%-2.89%)increase in the outpatient visits for acne.A 10μg/m^(3)increase in O_(3)8h concentration was associated with-0.62%(95%CI=-1.12%--0.11%)increase in the outpatient visits.Multipollutant models indicated that air pollutant concentrations were significantly associated with the daily outpatient visits for acne.Conclusions SO_(2)and NO_(2)are positively associated with daily dermatological department visits for acne in Lanzhou,while O_(3)8h concentration has a negative correlation with the visits.The sensitivity of acne daily outpatient visits to changes in SO_(2),NO_(2),and O_(3)8h varies among patients of different genders and at different ages.
