摘　　要：目的探讨Ilizarov组合式外固定器治疗合并跟腱挛缩的重度足踝部畸形的临床疗效。方法回顾性分析26例合并跟腱挛缩的足踝部畸形患者的临床资料,患者均采用Ilizarov组合式外固定器治疗,观察其临床疗效及预后。结果所有患者治疗时间为5~40周,平均18.6周,术后随访3~36个月,平均15.9个月。末次随访时患者美国足踝医师协会踝与后足关节(AOFAS)评分较术前明显改善(P<0.01)。参照AOFAS评分标准进行疗效评价,患者治疗优良率为96.2%。2例患者术后出现针道感染,经换药、局部注射抗菌药物后得到控制;无患者因感染等原因拔除钢针。结论Ilizarov组合式外固定器治疗合并跟腱挛缩的足踝部畸形疗效确切,且创伤小、安全性较高,能够保证矫形效果,促进功能恢复。Objective To explore the clinical efficacy of the Ilizarov combined external fixator in the treatment of severe foot and ankle deformities with Achilles tendon contracture.Methods The clinical data of 26 patients of foot and ankle deformity with Achilles tendon contracture were retrospectively analyzed.All the patients were treated with Ilizarov combined external fixator,and the clinical efficacy and prognosis were analyzed.Results The treatment time of all the patients was 5 to 40 weeks,with an average of 18.6 weeks,and the postoperative follow-up was 3 to 36 months,with an average of 15.9 months.At the last follow-up,American orthopedic foot and ankle society(AOFAS)scores of all the patients were significantly improved compared with those before operation(P<0.01).According to the AOFAS scoring standard,the efficacy was evaluated,the excellent and good rate was 96.2%.Needle tract infections occurred in 2 cases after operation,which were controlled after dressing changes and local injection of antibacterial drugs.No patient removed the steel needle for infection or other reasons.Conclusion Ilizarov combined external fixator is effective for the treatment of foot and ankle deformities with Achilles tendon contracture,which has the advantages of less trauma and high safety.It can ensure the orthopedic effect and promote functional recovery.
