医院+社区+家庭一体化健康管理预防2型糖尿病并发症的作用效果

摘　　要：目的:探究医院+社区+家庭一体化健康管理在2型糖尿病并发症预防中的作用。方法:2018年1-12月收治2型糖尿病患者100例,随机分为两组,各50例。两组入院时均接受常规诊治及护理;对照组出院后由专科护士定期电话随访;观察组给予医院+社区+家庭一体化健康管理。比较两组干预效果。结果:观察组干预后血糖控制达标率明显高于对照组,并发症发生率低于对照组,差异均有统计学意义(P<0.05);观察组干预后空腹血糖、餐后2 h血糖及糖化血红蛋白水平均低于对照组,差异有统计学意义(P<0.05);观察组干预后吸烟及饮酒的比例均低于对照组,合理饮食及合理运动的比例均高于对照组,差异有统计学意义(P<0.05)。结论:医院+社区+家庭一体化健康管理可有效提高血糖控制达标率,降低血糖水平,预防减少糖尿病并发症,并改善患者生活习惯。Objective:To explore the effect of integrated health management of hospital plus community plus family on prevention of complications of type 2 diabetes mellitus.Methods:From January to December 2018,100 patients with T2DM were selected and randomly divided into two groups with 50 cases in each group.Both groups received routine treatment and nursing on admission.The control group received regular telephone follow-up by specialist nurses after discharge.The observation group received hospital plus community plus family integrated health management.We compared the intervention effect of the two groups.Results:In the observation group,after the intervention,the standard rate of blood glucose control was significantly higher than that of the control group,and the incidence of complications was lower than that of the control group,the difference was statistically significant(P<0.05).In the observation group,after the intervention,fasting blood glucose,2 hours postprandial blood glucose and glycosylated hemoglobin levels were lower than those in the control group,the difference was statistically significant(P<0.05).In the observation group,after the intervention,the proportion of smoking and drinking was lower than that of the control group,and the proportion of reasonable diet and exercise was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Hospital plus community plus family integrated health management can effectively improve the blood glucose control compliance rate,reduce blood glucose level,prevent and reduce diabetic complications and improve the living habits of patients.
