复方苦参汤熏洗治疗混合痔术后并发症的疗效及对其血清TNF-α、IL-1、IL-6水平影响观察

摘　　要：目的观察复方苦参汤熏洗治疗混合痔术后并发症的疗效及对其血清TNF-α、IL-1、IL-6水平影响观察分析。方法将深圳平乐骨伤科医院(深圳市坪山区中医院)2019年3月至2020年8月收治拟择期行手术治疗的混合痔患者60例,采用随机数字表法分成实验组与对照组,每组各30例,其中对照组患者实施予聚维酮碘液坐浴,而实验组患者则以复方苦参汤熏洗治疗。比较两组患者术后并发症发生率,术后不同时间段创面指标(水肿、疼痛、渗出液评分)及术前、术后7 d时炎性指标(血清TNF-α、IL-1、IL-6水平)。结果实验组并发症发生率为3.33%低于对照组的26.67%,差异有统计学意义(P<0.05)。实验组T0~T5时水肿、疼痛、渗出液评分明显低于对照组,差异有统计学意义(P<0.05)。治疗前两组患者炎症指标比较,差异无统计学意义(P>0.05)。治疗后实验组患者炎症指标均低于对照组,差异有统计学意义(P<0.05)。结论在混合痔术后实施以复方苦参汤熏洗治疗,可明显减少术后并发症的发生率,减少术后不同时间段水肿、疼痛评分,并有效减少创面渗出液渗出,降低患者血清TNF-α、IL-1、IL-6的炎症因子水平,效果理想。Objective To observe the therapeutic efficacy of fumigating-washing therapy of compound sophorae decoction on postoperative complications of mixed hemorrhoids and its impact on serum TNF-α, IL-1 and IL-6 levels. Methods In this research, a total of 60 patients with mixed hemorrhoids as the diagnosis admitted and scheduled to undergo surgery in Shenzhen Pingle Orthopaedic Hospital(Shenzhen Pingshan District Traditional Chinese Medicine Hospital) recently(the period from March 2019 to August 2020) were divided into the experimental group(n=30) and the control group(n=30) according to random number table method. Patients in the control group were treated with povidone iodine sitz bath, while those in the experimental group were treated with fumigationwashing therapy of compound sophorae decoction. The incidence of postoperative complications, wound indexes(edema, pain, and exudate scores) at different time periods after operation and inflammatory index(serum TNF-α, IL-1, IL-6 levels) within before operation and 7 days after operation were compared between the two groups. Results The incidence of complications of the experimental group was 3.33%, lower than 26.67% of the control group, with statistically significant difference(P < 0.05). The scores of edema, pain and exudate in the experimental group from T0 to T5 were significantly lower than those in the control group, with statistically significant differences(P < 0.05). Before treatment, there were no statistically significant differences in inflammatory indexes between the two groups(P > 0.05). After treatment, the inflammatory indexes of the experimental group were lower than those of the control group, with statistically significant differences(P < 0.05). Conclusion After the operation of mixed hemorrhoids, fumigating-washing therapy with compound sophorae decoction can obviously reduce the incidence of postoperative complications, reduce the scores of edema and pain at different periods after operation, effectively reduce the exudation of wound surface, and
