摘　　要：目的:探究个案管理-服务团队模式在严重精神障碍患者医院与社区康复中对其服药依从性及躯体功能的作用。方法:2020年4-9月收治严重精神障碍患者120例,依据就诊先后顺序分为两组,各60例。对照组采用常规社区护理管理模式;观察组采用个案管理-服务团队模式。比较两组管理效果。结果:观察组社会功能缺陷程度评分显著低于对照组,差异有统计学意义(P<0.05);观察组服药依从率显著高于对照组,差异有统计学意义(P<0.05);观察组躯体功能、生理功能、心理功能和社会功能评分均显著高于对照组,差异有统计学意义(P<0.05)。结论:个案管理-服务团队模式能够减轻严重精神障碍患者社会功能缺损程度,提高患者服药依从性,改善患者躯体功能。Objective:To explore the effects of case management-service team model on medication compliance and physical function of patients with severe mental disorders in hospital and community rehabilitation.Methods:From April to September in 2020,120 patients with severe mental disorders were selected,and divided into two groups according to the order of treatment,with 60 cases in each group.The control group adopted the conventional community nursing management mode.The observation group adopted case management-service team mode.Comparison of two groups of management effect.Results:The social function deficit score of the observation group was significantly lower than the control group,with the difference was statistically significant(P<0.05).The adherence rate of observation group was significantly higher than control group,with the difference was statistically significant(P<0.05).The scores of physical function,physiological function,psychological function and social function in the observation group were significantly higher than the control group,with the differences were statistically significant(P<0.05).Conclusion:The case management-service team model can reduce the degree of social function deficit in patients with severe mental disorders,improve patients'medication compliance,and improve patients'physical function.
