摘 要:目的:研究在对妊娠期糖尿病(GDM)孕妇应用地塞米松促胎肺成熟时,采用不同的注射方式对母婴的血糖影响。方法:选取2014年6月至2018年4月在本院就诊并确诊为GDM的孕妇140例为研究对象,将其中105例纳入观察组,并随机分为羊膜腔注射组、肌肉注射组、静脉注射组,每组各35例;其余35例纳入空白对照组。对观察组按照相应的注射方式应用地塞米松促胎肺成熟,对空白对照组不以任何途径应用地塞米松。对各组研究对象治疗前后的空腹血糖数据进行比较。结果:观察组(羊膜腔注射组、肌肉注射组、静脉注射组)的GDM孕妇治疗前血糖值低于治疗后的血糖值,差异显著(P<0.05),其中以羊膜腔注射组较另两个注射组所引起的血糖升高幅度更小;空白对照组患者的空腹血糖数据在分娩前后差异不显著(P>0.05)。羊膜腔注射组GDM孕妇临床并发症明显少于肌肉注射组和静脉注射组,差异显著(P<0.05);肌肉注射组、静脉注射组的GDM孕妇临床并发症明显少于空白对照组,差异显著(P<0.05),有统计学意义。结论:对GDM孕妇采用羊膜腔注射地塞米松促胎肺成熟所导致的患者血糖升高幅度相对较小,并且新生儿的预后良好,在临床上值得推广和应用。Objective:To study the effect of dexamethasone on blood glucose of pregnant women and infants with gestational diabetes mellitus(GDM).Methods:105 pregnant women with GDM from June 2014 to April 2018 were selected as observation group.They were randomly divided into amniotic cavity injection group,intramuscular injection group and intravenous injection group.Another 35 pregnant women with GDM were selected as blank control group.The observation group was treated with dexamethasone to promote fetal lung maturation,while the blank control group was not treated with dexamethasone in any way.The data of fasting blood glucose in four groups before and after treatment were compared.Results:The blood glucose before treatment was significantly lower than that after treatment(P<0.05);the data of fasting blood glucose in blank control group were not significantly different before and after treatment(P>0.05).the clinical complications of GDM pregnant women in amniotic cavity injection group were significantly less than those in intramuscular injection group and intravenous injection group(P<0.05);the clinical complications of GDM pregnant women in intramuscular injection group and intravenous injection group were significantly less than those in blank control group(P<0.05),with statistical significance.Conclusion:Injection of dexamethasone into amniotic cavity will not lead to the increase of blood glucose,and the prognosis of newborns is good.It is worth popularizing and applying in clinic.
