摘　　要：目的探讨磺胺嘧啶银脂质水胶敷料治疗重症天疱疮的效果,并总结护理要点,为临床提供一种有效的换药方法。方法采用前后对照研究方法将2017年8月—2019年3月住院患者32例设为对照组,2019年4月—2020年8月住院患者28例设为试验组。对照组患者采用湿润烧伤膏油纱进行换药,每天1~2次;试验组患者采用磺胺嘧啶银脂质水胶敷料进行换药,大量渗液每天换药1次,中量渗液3d换药1次,少量渗液5d换药1次。比较两组患者住院期间疼痛评分、创面愈合时间、入院后继发皮肤感染、感染创面愈合时间、感染病例住院时间。结果两组患者住院期间创面疼痛评分、创面愈合时间、入院继发皮肤感染发生率、感染创面愈合时间和感染病例住院天数比较,均P<0.05,差异具有统计学意义,试验组创面疼痛评分较对照组低,创面愈合时间较对照组短,入院继发皮肤感染发生率较对照组低,感染创面愈合时间和住院天数较对照组短。结论重症天疱疮创面采用磺胺嘧啶银脂质水胶敷料换药较湿润烧伤膏油纱换药法,能减轻患者创面疼痛,较好控制创面感染,加快创面愈合,值得临床推广应用。Objective To investigate the effect of sulfadiazine silver lipid water gel dressing on severe pemphigus.Methods History control trial was adopted in this study.A total of 32 patients who were hospitalized from August 2017 to March 2019 were selected as the control group,and another group of 28 patients hospitalized from April 2019 to August 2020 as the trial group.In the control group,moisture burn ointment gauze was used for dressing,1-2 times a day,while sulfadiazine silver lipid water gel was used for dressing,once a day in case of a large amount of exudate,once in 3 days in case of a medium amount of exudate,and once in 5 days in case of a small amount of exudate.The two groups were compared in terms of wound pain score,wound healing time,incidence of secondary skin infection after admission,healing time of infected wound,and hospital stay.Results The wound pain score of the trial group was significantly lower than that of the control group(P<0.05).The wound healing time was significantly shorter than that of the control group(P<0.05).The incidence of secondary skin infection was significantly lower than that of the control group(P<0.05),and the healing time of infected wound and hospital stay of the experimental group were both significantly shorter than those of the control group(P<0.05).Conclusions Compared with mebo gauze,sulfadiazine silver lipid water gel is more effective in the treatment of severe pemphigus.It can relieve the wound pain,lower wound infection and promote wound healing,worthy of clinical spreading.
