当归四逆汤对老年TURP术后寒战(阳虚寒盛证)的临床研究

摘　　要：目的观察当归四逆汤治疗老年经尿道前列腺电切术(TURP)后寒战(阳虚寒盛证)的临床疗效。方法患者62例(寒战分级1~4级)采用随机数字表法分为治疗组与对照组各31例,对照组予常规治疗,治疗组加予当归四逆汤温服,疗程3 d。观察两组核心体温、平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、血氧饱和度(SpO_2)的变化及中医临床证候评分。结果治疗组核心温度的恢复效果显著优于对照组(P <0.01);治疗组治疗后MAR、HR、RR值低于对照组,SpO_2值高于对照组(P <0.01);治疗组改善寒战临床症状及中医证候评分的效果均优于对照组(P <0.05)。结论当归四逆汤治疗老年TURP术后寒战(阳虚寒盛证)疗效确切。Objective:To observe the clinical efficacy of the traditional Chinese medicine Danggui Sini Decoction in the treatment of elderly patients with chills(yang deficiency and cold excessiveness type)after transurethral resection of the prostate(TURP).Methods:A total of 62 patients(chills grade 1 to 4)were randomly divided into the treatment group and the control group according to random number table methods,31 cases in each group.The two groups were treated with Chinese medicine on the basis of the treatment of yang deficiency and cold excessiveness type chills after TURP.Danggui Sini Decoction was taken warmly,and the course of treatment was 3 days.The changes in core body temperature,mean arterial pressure(MAP),heart rate(HR),respiratory rate(RR),blood oxygen saturation(SpO2)and the clinical efficacy of chills and the scores of clinical syndromes of traditional Chinese medicine were observed respectively in the two groups.Results:The recovery effect of core temperature in the treatment group was significantly better than that of the control group(P<0.01);the MAR,HR,and RR values of the treatment group were lower than those of the control group,and the SpO2 value was higher than that of the control group(P<0.01);the clinical symptoms of chills and the scores of TCM syndromes in the treatment group were better than those of the control group(P<0.05).Conclusion:Traditional Chinese medicine Danggui Sini Decoction can significantly improve the clinical symptoms of elderly patients with chills(yang deficiency and cold excessiveness type)after TURP.
