养元护肾消癜方治疗过敏性紫癜肾炎急性期的疗效及对微循环指标、血清sICAM-1、sVCAM-1、IL-33、IL-8的影响

摘　　要：目的观察养元护肾消癜方治疗过敏性紫癜肾炎(HSPN)急性期的疗效及对微循环指标、血清可溶性血管细胞黏附分子-1(sVCAM-1)、可溶性细胞间黏附分子-1(sICAM-1)、白细胞介素-8(IL-8)、白细胞介素-33(IL-33)水平的影响。方法将HSPN急性期患者96例随机分为对照组与观察组各48例。对照组给予西药治疗,观察组给予西药联合养元护肾消癜方治疗。比较两组临床疗效、治疗前后症状评分、血清内皮素-1(ET-1)、血栓素-2(TXB2)、6-酮-前列腺素F-1α(6-KetoPGF-1α)、一氧化氮(NO)、sICAM-1、sVCAM-1、IL-33、IL-8水平。结果观察组治疗总有效率为97.92%,明显高于对照组的87.50%(P <0.05),观察组治疗后的症状评分显著低于对照组(P <0.05)。治疗后,观察组的血清6-Keto-PGF-1α、NO水平均高于对照组,血清TXB2、ET-1、sICAM-1、sVCAM-1、IL-33、IL-8水平均低于对照组(P <0.05),观察组的肾尿素氮(BUN)、血清肌酐(Scr)、尿红细胞计数及24 h尿白蛋白量(24 h UP)均低于对照组(P <0.05)。结论养元护肾消癜方治疗HSPN急性期可显著减轻临床症状及体征,改善微循环状态并抑制炎症状态,保护肾脏功能。Objective:To investigate the efficacy of Yangyuan Hushen XIaodian Prescription in the treatment of Henoch-Schonlein purpura nephritis(HSPN)at acute phase and its influence on microcirculation indicators,serum levels of soluble vascular cell adhesion molecule-1(sVCAM-1),soluble intercellular adhesion molecule-1(sICAM-1),interleukin-8(IL-8),interleukin-33(IL-33).Methods:A total of 96 patients with acute phase of HSPN were selected and randomly divided into two groups:the control group(n=48)was treated with western medicine,the observation group(n=48)was treated with western medicine combined with Yangyuan Hushen XIaodian Prescription.The clinical efficacy,syndrome scores,serum levels of endothelin-1(ET-1),thromboxane-2(TXB2),6-keto-prostaglandinF-1α(6-KetoPGF-1α),nitric oxide(NO),sICAM-1,sVCAM-1,IL-33 and IL-8 of the two groups were compared.Results:The total effective rate in the observation group was significantly improved compared with the control group(97.92%vs 87.50%,P<0.05);the symptoms scores in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of serum 6-Keto-PGF-1αand NO in the observation group were higher than those in the control group,and the levels of serum TXB2,ET-1,s ICAM-1,s VCAM-1,IL-33,and IL-8 were lower than the control group(P<0.05).After treatment,the blood urea nitrogen(BUN),serum creatinine(Scr),urinary red blood cell count and 24 hour urine albumin(24 h UP)in the observation group were all lower than those in the control group(P<0.05).Conclusion:Yangyuan Hushen XIaodian Prescription can significantly reduce clinical symptoms and signs of patients with HSPN at acute phase,which can improve the microcirculation state,suppress the inflammatory state,and protect renal function.
