摘  要:目的探讨优化后的UN2000尿液全自动化流水线系统在尿液红细胞检测中的应用价值。方法随机选取厦门市中医院2020年5—6月537例住院及门诊患者新鲜尿液作为研究标本,采用UN2000尿液全自动化流水线系统和显微镜离心镜检两种检测方法同时检测,统计分析其检测尿液红细胞的阳性符合率、阴性符合率、总符合率,分析出现假阳性、假阴性的原因,同时采用Kappa分析相差显微镜法和UN2000尿液全自动化流水线系统在尿液红细胞形态学分类信息的一致性。结果UN2000尿液全自动化流水线系统和显微镜镜检法检测尿液红细胞,差异有统计学意义(P<0.05)。两种方法检测红细胞阳性符合率为61.45%,阴性符合率为35.57%,总符合率为97.02%。显微镜检测尿液红细胞阳性率为61.82%;UN2000尿液全自动化流水线系统检测红细胞阳性率为64.06%,假阳性率为2.61%,假阴性率为0.37%。相差显微镜法和UN2000尿液全自动化流水线系统在尿液红细胞形态学的混合性与均一性的信息方面符合率较好,分别为91.25%和95.10%,在红细胞形态学的多形性、未提示信息方面符合率较差,分别为78.57%和86.57%。两种方法检测尿液红细胞形态学提示多形性、均一性、混合性、未提示红细胞信息分类一致性方面采用Kappa分析,Cohen’s Kappa系数为0.835,P<0.001,具有强一致性。结论UN2000尿液全自动化流水线系统可提高尿液红细胞检测的准确率,降低镜检率;运用UN2000尿液全自动化流水线系统检测尿液红细胞形态学信息分类与相差显微镜镜检法一致性强,与相差显微镜镜检相结合对尿液红细胞形态及其分型进行鉴别,有助于区分肾性和非肾性血尿患者,值得推广应用。Objective To explore the application value of the optimized UN2000 urine fully automated assembly line system in the detection of urine erythrocyte.Methods A total of 537 inpatients and outpatients in Xiamen Hospital of Traditional Chinese Medicine from May to June 2020 were selected as research objects.UN2000 urine fully automated assembly line system and the microscopic examination were used for the detection of urine erythrocyte.The positive coincidence rate,negative coincidence rate and total coincidence rate of urine erythrocyte were statistically analyzed,and the causes of false positive and false negative were analyzed.The Kappa analysis was used to analyze the consistency of phase contrast microscope method and UN2000 urine fully automated assembly line system in morphological classification of urine erythrocyte.Results There was significant difference between UN2000 urine fully automated assembly line system and microscopic examination method in detecting urine erythrocyte(P<0.05).The positive coincidence rate of the two methods was 61.45%,the negative coincidence rate was 35.57%,and the total coincidence rate was 97.02%.The positive rate of urine erythrocyte detected by microscope was 61.82%.The positive rate of erythrocyte detected by UN2000 urine fully automated assembly line system was 64.06%,the false positive rate was 2.61%,and the false negative rate was 0.37%.The phase contrast microscopy and UN2000 urine fully automated assembly line system have good coincidence rates in Mixed and Isomorphic information of urine erythrocyte morphology were 91.25%and 95.10%,respectively.The coincidence rate was poor in Dysmorphic and Non-classified information of urine erythrocyte morphology were 78.57%and 86.57%,respectively.The morphology of urine red blood cells detected by the two methods in the identification of Dysmorphic,Isomorphic,Mixed,and Non-classified.The Kappa analysis showed that the Cohen’s kappa coefficient was 0.835,P<0.001,which showed strong consistency.Conclusion UN2000 urine fully automated
