摘 要:目的探讨2型糖尿病患者胸主动脉钙化的危险因素及冠心病的相关性。方法回顾性研究334例因疑有冠心病而行冠脉造影的2型糖尿病患者,术前均行胸部CT检查,分析胸主动脉钙化的危险因素及其与冠心病的相关性。结果多因素非条件logistic回归分析显示糖尿病病程(OR=1.047,95%CI:1.010~1.085,P=0.012)、吸烟年限(OR=1.866,95%CI:1.039~3.352,P=0.037)、颈动脉硬化(OR=2.717,95%CI:1.602~4.610,P<0.001)为2型糖尿病胸主动脉钙化的独立危险因素;此外,2型糖尿病合并胸主动脉钙化是冠心病的独立危险因素(OR=2.970,95%CI:1.372~6.431,P=0.006);Pearson相关性分析显示2型糖尿病合并胸主动脉钙化与冠心病呈正相关(r=0.147,P=0.007),差异有统计学意义(P<0.05)。结论糖尿病及吸烟年限越长,或合并颈动脉硬化的2型糖尿病患者,胸主动脉钙化的发生风险更高;2型糖尿病合并胸主动脉钙化的患者是冠心病的高危人群。Objective To investigate the association between calcification of the thoracic aorta,cardiovascular risk factors and coronary artery disease in type 2 diabetic patients.Methods A retrospective study was conducted on 334 patients with type 2 diabetes who underwent coronary angiography due to suspected coronary heart disease.Preoperative chest CT examination was performed to analyze the risk factors of thoracic aorta calcification and its correlation with coronary artery disease.Results Multiple logistic regression analysis showed that duration of diabetes(OR=1.047,95%CI:1.010~1.085,P=0.012),smoking(OR=1.866,95%CI:1.039~3.352,P=0.037),carotid atherosclerosis(OR=2.717,95%CI:1.602~4.610,P<0.001)were independent risk factors for thoracic aorta calcification in type 2 diabetic patients.In addition,type 2 diabetes mellitus with thoracic aorta calcification was an independent risk factor for CAD(OR=2.970,95%CI:1.372~6.431,P=0.006).Pearson correlation analysis showed that there was a positive correlation between type 2 diabetes mellitus with thoracic aortic calcification and CAD(r=0.147,P=0.007),and the difference was statistically significant.Conclusion Patients with longer duration of diabetes and smoking years,or with carotid atherosclerosis,had a higher risk of thoracic aorta calcification.Type 2 diabetes mellitus with thoracic aorta calcification is a high risk group of coronary artery disease.
