永久性起搏器并发症的相关因素分析

摘　　要：目的:分析心脏永久性起搏器置入术后并发症以助预防和处理。方法:选取2009年2月~2020年9月复旦大学附属金山医院永久性心脏起搏器置入术患者620例,进行回顾性研究,分析并发症的原因及处理方法。结果:起搏器置入术后并发症共78例,包括感知障碍9例(11.5%)、电极脱位10例(12.8%)、囊袋破溃9例(11.5%)、囊袋血肿16例(20.5%)、起搏器感染11例(14.1%)、起搏器综合征12例(15.3%)、血胸和(或)气胸4例(5.1%)、膈肌和(或)局部肌肉跳动3例(3.8%)、电池耗竭提前4例(5.1%)。结论:起搏器术后并发症及时处理,减少并发症危害。应重视起搏器教育,危险因素改善,规范化操作和门诊规律随访。Objective:To explore the causes of complications after permanent cardiac pacemaker implantation,and provide a basis for preventing complications.Methods:From February 2009 to September 2020,620 patients undergoing permanent cardiac pacemaker implantation in Jinshan Hospital of Fudan University were selected for a retrospective study to analyze their complications.Results:There were 78 cases of complications after the permanent cardiac pacemaker implantation,10 cases(12.8%)of electrode displacement,9 cases(11.5%)of undersensing,3 cases(3.8%)of local muscle and diaphragm beating,16 cases(20.5%)of hematoma,9 cases(11.5%)of pocket rankle,11 cases(14.1%)of pacemaker infection,12 cases(15.3%)of pacemaker syndrome,4 cases(5.1%)of hemothorax and pneumothorax,4 cases(5.1%)of battery exhaust advance.Conclusion:Find out the complications after the pacemaker in time,analyze the causes and deal with them immediately.In order to reduce the occurrence of complications,we must pay attention to the risk factors of complications and prevent them,promote pacemaker publicity and education,standardize intraoperative operations,and strengthen postoperative follow-up.
