摘　　要：目的分析Claudin-4在胆囊癌(GBC)组织中的表达及其与患者临床病理特征、预后的关系。方法收集2014年1月至2018年12月间于蚌埠医学院第一附属医院行根治性手术的50例GBC患者的肿瘤标本及临床资料,并定期随访,选择同期入院的20例胆囊腺瘤及20例慢性胆囊炎作为对照;免疫组化法检测胆囊组织中Claudin-4的表达,分析其与GBC患者临床病理特征、预后的关系;Keplan-Meier(KM)生存曲线、log-rank检验及Cox回归分析探讨Claudin-4对患者总体生存率(OS)的影响。结果GBC患者肿瘤组织中Claudin-4的表达水平显著高于胆囊炎和胆囊腺瘤(P<0.05);GBC患者Claudin-4表达与血清糖类抗原19-9(CA19-9)水平、肿瘤分化程度显著相关;KM生存曲线提示,Claudin-4表达阳性的患者有较差的OS;Cox多因素分析提示,Claudin-4是GBC的独立预后因素。结论Claudin-4在GBC组织中高表达,与患者血清CA19-9、肿瘤分化程度密切相关,并影响患者术后OS。Objective To investigate the expression of Claudin-4 in gallbladder carcinoma(GBC)and its relation to clinicopathological features and prognosis.Methods The tumor samples and clinical data of 50 GBC patients who underwent radical surgery in our hospital from January 2014 to December 2018 were collected and regularly followed up.In addition,20 patients with gallbladder adenoma and 20 patients with chronic cholecystitis were selected as control groups.Immunohistochemical method was used to detect the expression of Claudin-4 in tissues,and the relationship between Claudin-4 and the clinicopathological features of GBC patients was analyzed.Keplan-Meier(KM)survival curve,log-rank test and Cox regression analysis were used to assess the effect of Claudin-4 on the overall survival(OS).Results The expression of Claudin-4 in tumor tissues of GBC patients was significantly higher than that in tissues of cholecystitis and gallbladder adenoma,and the difference was statistically significant(P<0.05).In GBC patients,the level of Claudin-4 was significantly correlated with the level of serum carbohydrate antigen 19-9(CA19-9)and tumor differentiation.KM survival curve showed that patients with the positive expression of Claudin-4 had worse OS.Cox multivariate analysis showed that Claudin-4 was an independent prognostic factor for GBC.Conclusion Claudin-4 is highly expressed in GBC,is closely related to serum CA19-9 and tumor differentiation,and significantly affects the postoperative OS.
