鼻腔填塞法和贯穿连续缝合法应用于鼻中隔偏曲矫正手术后的效果分析

摘　　要：目的探讨鼻腔填塞法和贯穿连续缝合法应用于鼻中隔偏曲矫正手术后的效果。方法选取复旦大学附属眼耳鼻喉科医院2017年12月至2019年10月特需病房耳鼻喉科收治行鼻中隔偏曲矫正手术患者共120例,根据抽签法随机分为缝合组(行贯穿连续缝合法)60例和填塞组(行高分子膨胀海绵填塞法)60例。比较两组患者术后48 h的主观症状视觉模拟量表(VAS)评分及术后2周并发症发生率。结果填塞组患者术后48 h的视觉模拟量表评分均值大于缝合组(P<0.05)。术后2周门诊复诊鼻内镜检查两组患者,术后均未出现鼻出血、鼻中隔穿孔、鼻中隔血肿等并发症,但填塞组有2例出现鼻腔粘连,2例出现鼻腔异味,而缝合组1例出现鼻腔粘连,1例出现鼻腔异味。结论相比较于鼻腔填塞法,贯穿连续缝合法能明显减轻鼻中隔偏曲患者术后不适感,减轻患者痛苦,同时不会增加患者手术后并发症。Objective To investigate the effects of nasal packing and continuous suture on the symptoms and complications of nasal septum correction.Methods A total of 120 patients with nasal septum deviation treated in Department of Otolaryngology of Fudan University Affiliated Department of Otolaryngology from December 2017 to October 2019 were selected and randomly divided into suture group(with continuous suture method)and packing group(with polymer expanded sponge packing method)with 60 cases each group.The visual analogue scale(VAS)score of subjective symptoms and the incidence of complications at 2 weeks after operation were compared between the two groups.Results The VAS score of the packing group was higher than that of the suture group 48 hours after operation(P<0.05).Two weeks after the surgery,there were no postoperative complications such as epistaxis,nasal septum perforation,nasal septum hematoma in two group,but in the packing group,there were 2 cases of nasal adhesion,2 cases of nasal odor,and 1 case of nasal adhesion and 1 case of nasal odor in the suture group.Conclusions Continuous suture can significantly reduce postoperative discomfort and pain of patients with deviated nasal septum without increasing postoperative complications.
