芒针深刺环跳穴辅以龙虎交战手法治疗慢性下肢缺血性疼痛的临床研究

摘　　要：目的观察芒针深刺环跳穴辅以龙虎交战手法治疗慢性下肢缺血性疼痛的临床疗效。方法将2017年7月—2019年6月石家庄市中医院治疗的100例慢性下肢缺血性疼痛患者随机分为2组,每组50例,对照组给予常规西药治疗,观察组在其基础上给予芒针深刺环跳穴辅以龙虎交战手法治疗,隔日治疗1次,共治疗14 d。观察2组干预后临床症状评分、最大步行及无痛行走距离、踝肱指数(ABI)、足背动脉、腘动脉血流动力学参数、生活质量的变化,比较2组临床疗效。结果治疗后2组疼痛视觉模拟评分(VAS)、下肢麻木评分、皮肤色泽评分、皮肤温度感评分及血管生存质量量表(VascuQOI)各维度(疼痛、其余症状、日常生活、社交活动、情感问题)评分及总评分均明显降低(P均<0.05),且治疗后观察组上述评分均低于对照组(P均<0.05)。与治疗前比较,治疗后2组最大步行距离、无痛行走距离、ABI及足背动脉、腘动脉的血流量与血管内径均明显增加(P均<0.05),且治疗后观察组上述指标均高于对照组(P均<0.05)。观察组总有效率为90%(45/50),对照组为72%(36/50),观察组明显高于对照组(P<0.05)。结论芒针深刺环跳穴辅以龙虎交战手法治疗慢性下肢缺血性疼痛疗效确定,不仅可有效缓解下肢缺血性疼痛,改善下肢血液循环,还能提高最大步行及无痛行走距离,改善生活质量。Objective It is to observe the clinical curative effect of deep acupuncture at Huantiao acupoint with dragon-tiger engaging manipulation on chronic lower limb ischemic pain.Methods One hundred patients with chronic lower limb ischemic pain treated in Shijiazhuang Hospital of Traditional Chinese Medicine from July 2017 to June 2019 were randomly divided into 2 groups,50 cases in each group.The control group was treated with deep acupuncture at Huantiao acupoint with dragon-tiger engaging manipulation which was was performed every other day based on the control group.Both groups were treated for a total of 14 days.The changes in clinical symptom score,maximum walking distance and painless walking distance,ankle-brachial index(ABI),hemodynamic parameters of dorsal foot artery and popliteal artery,and quality of life after the intervention were observed in the two groups,and the clinical effects of the two groups were compared.Results After treatment,The scores of pain visual analogue score(VAS),lower limb numbness,skin color,skin temperature feeling,and the scores of each item(pain,other symptoms,daily life,social activities,emotional problems)and total scores in VascuQOI scale were significantly reduced(all P<0.05),and the above scores of the observation group were lower than those of the control group after treatment(P<0.05).Compared with before treatment,the maximum walking distance,painless walking distance,ABI,blood flow and vascular inner diameter of dorsal foot artery and popliteal artery of the two groups were significantly increased after treatment(all P<0.05),and the above indicators in the observation group were all higher than those of the control group after treatment(P<0.05).The total effective rate of the observation group was 90%(45/50),and that of the control group was 72%(36/50),the total effective rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Deep acupuncture at Huantiao acupoint with dragon-tiger engaging manipulation can not only effecti
