摘　　要：目的探究有创-无创序贯性机械通气治疗手术后呼吸衰竭患者的气道管理效果。方法回顾性分析2019年3月—2020年9月就诊的120例手术后呼吸衰竭患者,根据治疗方法将其分为研究组(n=60)与对照组(n=60)。研究组采用有创-无创序贯性机械通气治疗,对照组采用有创机械通气治疗,比较2组患者的疗效。结果2组患者治疗后24 h的PaCO_(2)水平低于治疗前,PaO_(2)、SaO_(2)水平高于治疗前,且研究组改善幅度大于对照组,差异有统计学意义(P<0.05);研究组的再插管率、住院时间显著低于对照组,差异有统计学意义(P<0.05)。结论有创-无创序贯性机械通气治疗可以有效缓解手术后呼吸衰竭患者的缺氧情况,降低再插管率,促进术后恢复。Objective To explore the effect of invasive noninvasive sequential mechanical ventilation on airway management in patients with postoperative respiratory failure.Methods 120 patients with respiratory failure after operation in our hospital from March 2019 to September 2020 were retrospectively analyzed.According to the treatment methods,they were divided into study group(n=60)and control group(n=60),The study group was treated with invasive noninvasive sequential mechanical ventilation;the control group was treated with invasive mechanical ventilation.The therapeutic effects of the two groups were detected and compared.Results Repeated measurement results showed that the level of PaCO_(2)24 hours after treatment in the two groups was lower than that before treatment,and the levels of PaO_(2) and SaO_(2) were higher than that before treatment,the difference was statistically significant(P<0.05);the level of PaCO_(2)24 hours after treatment in the study group was lower than that in the control group,and the levels of PaO_(2) and SaO_(2) were higher than that in the control group,the difference was statistically significant(P<0.05);the reintubation rate and hospitalization time in the study group were significantly lower than those in the control group,the difference was statistically significant(P<0.05)The difference was statistically significant(P<0.05).Conclusion Invasive noninvasive sequential mechanical ventilation can effectively alleviate the hypoxia of postoperative respiratory failure,reduce the rate of re intubation,and promote the recovery of surgery.
