摘 要:目的通过比对分析化学发光法和连续监测法测定血清脂蛋白相关磷脂酶A2(Lipoprotein-Associated Phospholipase A2,Lp-PLA2)的各项性能指标,评价两种方法学是否能符合临床实验室要求而应用于临床。方法收集冠状动脉粥样硬化患者样本248例以及健康人样本52例,用化学发光法和连续监测法分别检测血清脂蛋白相关磷脂酶A2(Lp-PLA2),并对2组结果进行比对分析,评估临床符合率及相关性。结果化学发光法的精密度和准确度(回收率)分别为3.49%,2.17%(<5%)和101.93%(90%~110%),连续监测法的精密度和准确度(相对偏差)分别为1.82%,1.54%(<2.5%)和-2.49%,-1.24%(<±5%),化学发光法和连续监测法检测冠状动脉粥样硬化标本的临床符合率分别为88.31%和81.85%,健康人群标本的临床符合率分别为100%和98.08%,两种方法学的相关系数方程为Y=0.4495X-30.225,相关系数r2=0.972,具有良好的相关性。结论化学发光法和连续监测法在测定Lp-PLA2中各项性能指标均符合临床实验室要求,连续监测法具有操作简单、成本低等优势,可满足临床大批量常规样本检测,而化学发光法在原理及临床符合率方面要优于连续监测法。Objective To determine the performance indicators of serum lipoprotein-associated phospholipase A2(Lp-PLA2)by comparison analysis chemiluminescence method and continuous monitoring method Clinical requirements.Methods Comparison of evaluation principles,precision and accuracy of the two methodologies were carried out.248 patients with coronary atherosclerosis and 52 healthy people were collected,and serum lipoprotein-related phospholipids were detected by the two methods.Enzyme A2(Lp-PLA2),and comparative analysis of the results of the two groups to evaluate the clinical coincidence rate and correlation.Results The precision and accuracy(recovery rate)of the chemiluminescence method were 3.49%,2.17%(<5%)and 101.93(90%-110%),respectively.The precision and accuracy of the continuous monitoring method(relative deviation)1.82%,1.54%(<2.5%)and-2.49%,-1.24%(<±5%)respectively.The clinical coincidence rates of coronary atherosclerosis samples detected by chemiluminescence and continuous monitoring methods were 88.31% and 81.85%,the clinical coincidence rates of healthy population specimens are 100% and 98.08%,the correlation coefficient equation of the two methodologies is Y=0.4495x-30.225,the correlation coefficient R2=0.972(>0.95),the results have a good correlation.Conclusion Chemiluminescence method and continuous monitoring method in the determination of Lp-PLA2 all performance indicators meet the requirements of clinical laboratories.The continuous monitoring method has the advantages of simple operation and low cost,which can satisfy the clinical large-scale routine sample detection,and the chemiluminescence method is superior to the continuous monitoring method in terms of principle and clinical compliance rate.
