摘　　要：目的:探索医院实施处方前置审核系统的效果,为临床合理用药提供参考。方法:选取河南省人民医院2018年1-6月处方前置审核系统上线前的处方和2019年1-6月、2020年1-6月处方前置审核系统上线后的门诊处方,分析三个时间段的处方合理率以及不合理处方类型的变化。结果:与2018年1-6月相比,2019年1-6月以及2020年1-6月门诊处方的平均合理率显著提升。“开具处方未写临床诊断或临床诊断书写不全”、“遴选药品不适宜”、“药品剂型或给药途径不适宜”和“用法、用量不适宜”的不合理处方在门诊处方中的占比随着前置审方系统的完善明显降低;同时,一些不合理问题如“适应证不适宜”和“不良药物相互作用”随着系统的完善更多地显现出来。结论:处方前置审核系统的建立有效地提高了本院门诊处方合理率和处方质量。Objective:To explore the effect of pre-prescription review system,so as to provide reference for rational use of drugs in clinic.Methods:The prescriptions in the outpatient department of Henan Provincial People’s Hospital from January to June in 2018(without the on-line pre-prescription review system)and the prescriptions from January to June in 2019 and 2020(with the on-line pre-prescription review system)were collected,and the rate of reasonable prescriptions and the changes of unreasonable prescription types were analyzed during the above-mentioned time spans.Results:The rate of reasonable prescriptions from January to June in 2019 and 2020 in the outpatient department significantly increased as compared with that of the time span of 2018.The rates of unreasonable prescriptions,including“prescriptions without clinical diagnosis or with incomplete clinical diagnosis”,“prescriptions with unsuitable drugs”,“prescriptions with unsuitable dosage forms or routes of administration”and“prescriptions with unsuitable applications or dosages”reduced significantly during the time spans from January to June in 2019 and 2020 compared with the time span from January to June in 2018.At the same time,such problems as“prescriptions with unsuitable indications”and“adverse drug interactions”appeared more frequently with the improvement of the review system.Conclusion:The establishment of the pre-prescription review system significantly improved the rate and quality of reasonable prescriptions in the outpatient department of the hospital.
