重症监护病房中冠心病伴发心律失常患者住院时间的影响因素分析

摘　　要：目的:探讨重症监护病房中冠心病伴发心律失常患者住院时间的影响因素。方法:收集2017年2月至2019年4月于新乡市中心医院冠心病重症监护病房(Coronary care unit,CCU)治疗的335例患者临床资料,根据是否伴发室性心律失常(Ventricular arrhythmias,VA)进行分组,其中伴发VA患者59例,无VA患者276例。采用单因素分析和Logistics回归分析对影响患者住院时间的因素进行统计分析。结果:CCU患者伴发VA患者相比无VA患者年龄增大,血脂异常和糖尿病患者人数增加,住院时间延长(P<0.05)。以住院时间为因变量,各临床资料为自变量纳入Logistic逐步回归模型,结果显示年龄、血脂异常、糖尿病以及左室射血分数是影响VA患者住院时间的相关因素(P<0.05),伴有血脂异常、糖尿病以及左室射血分数<40%的VA患者住院时间明显延长,而年龄则与VA患者CCU住院时间呈相关(r=0.598,P<0.01)。结论:VA是延长CCU患者的住院时间的主要原因,老年、血脂异常、糖尿病以及心功能是影响因素。Objective:To investigate the influencing factors of hospitalization duration in patients with coronary heart disease accompanied by arrhythmia in intensive care unit.Methods:To collect clinical data of 335 patients treated in Coronary Care Unit(CCU)of Xinxiang City Central Hospital from February 2017 to April 2019,and grouped according to whether accompanied by Ventricular arrhythmias(VA),including 59 cases of patients with VA,276 cases of patients without VA.Univariate analysis and logistics regression analysis were used to analyze the factors affecting patients'duration of hospitalization.Results:Compared with patients without VA,CCU patients with VA were increased in age,the number of patients with dyslipidemia and diabetes,and the length of hospital stay was prolonged,and the differences were statistically significant(P<0.05).The length of time as the dependent variable,the clinical data for the independent variables into the Logistic regression model,the results showed that age,dyslipidemia,diabetes and left ventricular ejection fraction was the related factors influencing the CCU days of VA patients(P<0.05),the CCU days of patients with dyslipidemia,diabetes and left ventricular ejection fraction less than 40%were significantly longer,while the age of CCU patients with VA was correlated with CCU days(r=0.598,P<0.01).Conclusion:VA was the main reason to prolong the hospital stay of CCU patients,and the elderly,dyslipidemia,diabetes and cardiac function were the influencing factors.
