不同时机膝关节镜下前交叉韧带重建术的临床研究

摘　　要：目的:分析不同时机行膝关节镜下前交叉韧带重建术治疗前交叉韧带损伤的效果。方法:选取2017~2019年收治的102例前交叉韧带损伤患者,根据受伤至手术时间进行分组,其中≤3周为早期组(n=52),>3周为晚期组(n=50),对比两组手术效果。结果:两组术后Lysholm评分、国际膝关节文献委员会膝关节评估表评分、膝关节活动度较术前提升(P<0.001),早期组各指标水平较晚期组获得更大改善(P<0.001);早期组疼痛视觉模拟评分低于晚期组(P<0.001),早期组骨隧道宽度、信号强度、腱骨结点T2值优于晚期组(P<0.001)。结论:早期(≤3周)行膝关节镜下前交叉韧带重建术治疗前交叉韧带损伤的效果更为理想,值得临床推广。Objective:To analyze the effects of arthroscopic anterior cruciate ligament reconstruction at different time in the treatment of anterior cruciate ligament injury.Methods:From 2017 to 2019,102 cases of anterior cruciate ligament injury were selected.Patients were divided into two groups according to the time from injury to operation.52 patients were in the early surgery group(≤3 weeks),and 50 patients were in the late surgery group(>3 weeks).The surgical effects of the two groups were compared.Results:Postoperative Lysholm score,the international knee documentation committee knee evaluation form score and knee joint motion were significantly improved in both groups compared with those before surgery(P<0.001),and all indexes in the early surgery group were better improved than those in the late surgery group(P<0.001).The pain visual analogue scale score of the early surgery group was significantly lower than that of the late surgery group(P<0.001),and the width of the bone tunnel,the signal intensity and the T2 value of the tendon-bone junction in the early surgery group were significantly better than those in the late surgery group(P<0.001).Conclusions:Arthroscopic anterior cruciate ligament reconstruction at the early stage(≤3 weeks)is more effective in the treatment of anterior cruciate ligament injury and can be popularized.
