摘  要:目的研究江苏省A群链球菌的耐药性、耐药基因和分子分型的特征联系。方法收集2016年-2018年全省A群链球菌50株,采用微量肉汤稀释法测定12种抗生素最小抑菌浓度,采用PCR方法检测7种不同的耐药基因和转座子Tn916,并采用emm分型、多位点序列分型(MLST)和sof分型方法进行分子分型。结果 50株GAS对四环素、红霉素和克林霉素耐药率较高,对其他9种抗生素均敏感。耐药基因tet M、erm B及转座子Tn916携带率较高,未发现携带tet L的菌株,其他耐药基因携带率较低。emm分型分为5个emm型别,MLST分型分为5个ST型,有25株菌株可以扩增出sof基因,分为3个OF型。引起猩红热的GAS其耐药率、耐药种数和耐药基因携带数均高于引起其他症状的GAS(P <0.05)。主要分子型别之间(emm1和emm12,ST28和ST36,OF可分型和OF不可分型)其转座子Tn916携带率有差异。结论江苏地区GAS对四环素、红霉素和克林霉素耐药率较高,以携带耐药基因tet M和erm B为主,菌株主要emm型别为emm1和emm12,主要ST型别为ST28和ST36,主要OF型为12。Objective To understand the relation of characteristics among drug resistance,resistant genes and genotype of group A streptococci in Jiangsu Province. Method A total of 50 group A streptococci strains were collected from hospitals between 2016 and 2018. Twelve antimicrobial minimal inhibitory concentrations were detected by method of micro-dilution broth. Seven different resistance genes were detected by PCR. And all isolates were typed by emm typing,multiple sequence typing and sof typing. Results It shows that 50 GAS strains have high drug-resistance rate to erythromycin,clindamycin and tetracycline,though all the strains are sensitive to other 9 antibiotics. The carriage rate of tet M,erm B and Tn916 are slightly higher than tet S,tet A,erm TM and mef A. None of the strain carrying tet L is found. The GAS strains are classified to 5 emm types and 5 ST types. Half of GAS strains carry gene sof which were typed three OF types. The strains leading to scarflet fever had higher drug resistance rate,more resistant drugs and drug resistant genes types than those strains which cause other clinical symptom( P < 0. 05). There was significant difference in carriage of Tn916 between major genotypes( emm1 and emm12,ST28 and ST36,OF typed strains and OF none-typed strains). Conclusion GAS strains in Jiangsu Province are high resistant to erythromycin,clindamycin and tetracycline. The major drug resistant genes are tet M and erm B. The major emm types are emm1 and emm12,the major ST types are ST28 and ST36,and the major OF type is OF12.
