1682例不育症患者精液支原体检测及耐药性分析

摘　　要：目的探讨男性不育症患者精液支原体感染及药敏情况并对耐药进行分析,为临床治疗感染提供参考。方法回顾性调查3年内1 682例男性不育症患者精液支原体分离培养及药物敏感试验。结果 1 682例患者中精液支原体培养阳性240例,其中单一Uu <104cfu/spec有228例(95.00%),单一Uu≥104cfu/spec有8例(3.34%),Uu≥104cfu/spec+Mh <104cfu/spec有2例(0.83%),Uu <104cfu/spec+Mh <104cfu/spec有2例(0.83%)。单一Uu <104cfu/spec明显高于单一Uu≥104cfu/spec及Uu合并Mh混合感染(P <0.05);Uu及Mh对交沙霉素、原始霉素完全敏感,对强力霉素、四环素、克拉霉素、阿奇霉素、红霉素敏感性较高,其敏感性明显高于环丙沙星、氧氟沙星(P <0.05)。结论男性不育症患者精液支原体感染以单一Uu <104cfu/spec最为普遍,治疗首选交沙霉素,次选强力霉素,最好根据药敏试验选择药物。Objective To investigate the semen mycoplasma infection and drug sensitivity in patients with male infertility and to analyze the drug resistance,so as to provide reference for clinical treatment of infection. Methods Culture and drug sensitivity test of mycoplasma in semen of 1 682 patients with male infertility in three years were performed retrospectively. Results Among the 1 682 patients,240 cases were positive for semen mycoplasma culture,including 228 cases of single Uu < 104 cfu/spec( 95. 00%),8 cases of single Uu≥104 cfu/spec( 3. 34%),and 2 cases of Uu≥104 cfu/spec + Mh < 104 cfu/spec( 0. 83%),Uu < 104 cfu/spec + Mh < 104 cfu/spec( 0. 83%). Single Uu < 104 cfu/spec was significantly higher than single Uu ≥104 cfu/spec and Uu combined with Mh infection( P < 0. 05);Uu and Mh were completely sensitive to josamycin and protomycin,but more sensitive to doxycycline,tetracycline,clarithromycin,azithromycin and erythromycin,and their sensitivity was significantly higher than that of ciprofloxacin and ofloxacin( P < 0. 05). Conclusion Single Uu < 104 cfu/spec is the most common method for semen mycoplasma infection in male infertile patients,Josamycin is the first choice for treatment,doxycycline is the second choice,and drugs should be selected according to drug sensitivity test.
