HBeAg与抗-HBe同时表达的慢性乙型肝炎患者临床实验分析

摘　　要：目的分析慢性乙型肝炎(CHB)患者HBeAg与抗-HBe同时表达时的临床特点。方法选择2018年1月-2019年9月在本院就诊的CHB患者1250例,根据HBeAg的表达情况将其分为HBeAg与抗-HBe同时表达375例,HBeAg表达392例,抗-HBe表达483例,分析3组HBeAg、抗-HBe、HBV DNA以及肝功能指标ALT、AST水平的差异性。结果HBeAg与抗-HBe同时表达72.53%患者HBeAg水平在1 S/CO~20.0 S/CO,20.0 S/CO~100 S/CO占18.13%,>100 S/CO为9.34%,与HBeAg表达比较差异有统计学意义(P<0.05);HBeAg与抗-HBe同时表达13.33%患者抗-HBe水平在0 S/CO~0.1 S/CO,0.1 S/CO~0.5 S/CO占24.00%,0.5 S/CO~1.0 S/CO占62.67%,与抗-HBe表达比较差异有统计学意义(P<0.05);HBeAg与抗-HBe同时表达HBV DNA<30 IU/ml占30.13%,30 IU/ml~1×10^(3)IU/ml占45.67%,>1×10^(3)IU/ml占24.20%;HBeAg与抗-HBe同时表达ALT、AST的异常率均高于HBeAg表达和抗-HBe表达(P<0.05)。结论HBeAg与-HBe同时表达且HBV DNA高水平复制时,具有较强传染力且伴有严重肝损伤,应引起临床足够重视。Objective To analyze the clinical characteristics of the simultaneous expression of HBeAg and anti-HBe in patients with chronic hepatitis B(CHB).Methods A total of 1250 patients with chronic hepatitis B from January 2018 to September 2019 were selected.According to the expression of HBeAg,the patients were divided into 375 cases with HBeAg and anti-HBE expression,392 cases with HBeAg expression,and 483 cases with anti-HBE expression.The differences of HBeAg,anti-HBe,HBV DNA and liver function indexes ALT and AST levels in the three groups were analyzed.Results The HBeAg level of 72.53%patients with HBeAg and anti-HBe expression was 1 S/CO-20.0 S/CO,20.0 S/CO-100 S/CO accounted for 18.13%,that>100 S/CO was 9.34%,and there was statistical significance compared with HBeAg expression(P<0.05).In 13.33%patients with HBeAg and anti-HBE expression,the anti-HBE level was 0 S/CO-0.1 S/CO,0.1 S/CO-0.5 S/CO accounted for 24.00%,and 0.5 S/CO-1.0 S/CO accounted for 62.67%,which had statistical significance compared with the anti-HBE expression(P<0.05).HBV DNA<30 IU/ml was 30.13%,30 IU/ml-1×10^(3)IU/ml was 45.67%,>1×10^(3)IU/ml was 24.20%.The abnormal rates of ALT and AST expressed simultaneously by HBeAg and anti-HBe were higher than those expressed by HBeAg and anti-HBe(P<0.05).Conclusion When HBeAg and anti-HBE are simultaneously expressed and HBV DNA is duplicated at a high level,it has strong infectivity and is accompanied by severe liver injury,which should be paid enough attention in clinic.
