摘　　要：目的探讨射血分数保留心力衰竭(HFpEF)患者血清脂蛋白相关磷脂酶A_(2)(LP-PLA_(2))与心功能的相关性。方法选取2018年7月至2019年9月在郑州大学第五附属医院心内科初次收治的97例射血分数保留心力衰竭(HFpEF)患者为研究组,按照NYHA分级将研究组分为NYHAⅣ级组30例,NYHAⅢ级组34例,NYHAⅡ级组33例。另选取同期36例心功能正常者为对照组。检测血清LP-PLA_(2)水平、BNP水平,行心脏超声检查,用Spearman相关分析法分析LP-PLA_(2)水平与HFpEF患者BNP水平、左心室质量指数(LVMI)、NYHA心功能分级的相关性。结果NYHAⅡ、Ⅲ、Ⅳ级3组患者的LP-PLA_(2)水平均高于对照组(P<0.05),且NYHA分级越高,LP-PLA_(2)水平越高,差异有统计学意义(P<0.05)。Spearman相关性分析显示HFpEF患者LP-PLA_(2)水平与BNP水平、LVMI、NYHA心功能分级均呈正相关(r=0.576、0.570、0.626,P<0.05)。结论HFpEF患者血清LP-PLA_(2)水平升高,并且其与HF的严重程度密切相关,可能参与心室重构,对HFpEF的诊断和严重程度的评估有一定的临床参考价值。Objective To explore the relationship between the lipoprotein associated phospholipase A_(2)(LP-PLA_(2))and cardiac function in heart failure with preserved ejection fraction(HFpEF)patients.Methods A total of 97 patients with HFpEF initially admitted to the department of cardiology in the the Fifth Affiliated Hospital of Zhengzhou University from July 2018 to september 2019 were selected as the research group.According to the NYHA classification,the research group was divided into NYHAⅣgroup of 30 cases,NYHAⅢgroup of 34 cases and NYHAⅡgroup 33 cases.A total of 36 patients with normal cardiac function were selected as the control group.The serum levels of LP-PLA_(2)and BNP were tested.The echocardiography examination was conducted.The correlation of LP-PLA_(2)with BNP,LVMI,and NYHA classification in HFpEF patients was analyzed by the Spearman relevance analysis.Results The serum level of LP-PLA_(2)in NYHAⅣgroup,NYHAⅢgroup and NYHAⅡgroup were higher than those in the control group(P<0.05).With the increase of the NYHA classification,the serum level of LP-PLA_(2)was increased(P<0.05).Spearman relevance analysis showed that serum LP-PLA_(2)had a positive correlation with BNP,LVMI and NYHA classification in HFpEF patients(r=0.576、0.570、0.626,P<0.05).Conclusion The serum level of LP-PLA_(2)in HFpEF patients is increased and closely correlated with severity of HF.It may be involved in ventircular remodeling,which can provide the important clinical value for the diagnosis and severity judgment of HFpEF.
