综合保温护理对糖尿病全麻腹部手术患者低体温的预防作用

摘　　要：目的探讨综合保温护理对糖尿病全麻腹部手术患者低体温的预防作用。方法选择2019年2月—2020年1月该院120例糖尿病腹腔镜全麻腹部手术患者作为研究对象,以随机数字表法将其随机分为两组,其中对照组与研究组各60例。对照组术中采取常规保温护理措施,研究组在此基础上应用综合保温护理措施。对比两组术前与术后即刻的血糖指标、术中低体温、术后躁动与寒战的发生率,以及术后拔管时间与苏醒时间。结果研究组术后即刻血糖值(5.9±0.5)mmol/L低于对照组(6.5±0.4)mmol/L,差异有统计学意义(t=7.258,P<0.01)。研究组术中低体温与术后寒战的发生率1.67%、0.00%均低于对照组13.33%、10.00%,差异有统计学意义(χ^(2)=4.324、4.386,P<0.05)。研究组患者拔管时间(29.6±5.6)min与苏醒时间(17.5±6.5)min低于对照组(45.6±5.4)min、(26.6±4.8)min,差异有统计学意义(t=15.391、8.724,P<0.01)。结论综合保温护理可以有效减少糖尿病全麻腹部手术患者血糖指标的波动,降低低体温与寒战的发生率,加快术后恢复速度,适于临床应用与推广。Objective To explore the preventive effect of comprehensive insulation nursing on hypothermia in diabetic patients undergoing general anesthesia abdominal surgery.Methods From February 2019 to January 2020,120 diabetic patients with laparoscopic general anesthesia abdominal surgery in the hospital were selected as the research objects,and they were randomly divided into two groups by random number table method,of which the control group and the study group had 60 cases each.The control group took routine heat preservation nursing measures during the operation,and the study group applied comprehensive heat preservation nursing measures on this basis.The preoperative and immediate postoperative blood glucose indicators,intraoperative hypothermia,postoperative restlessness and chills,as well as postoperative extubation time and recovery time were compared between the two groups.Results The immediate postoperative blood glucose level(5.9±0.5)mmol/L of the study group was lower than that of the control group(6.5±0.4)mmol/L,the difference was statistically significant(t=7.258,P<0.01).The incidences of intraoperative hypothermia and postoperative chills in the study group were 1.67%and 0.00%lower than those in the control group 13.33%and 10.00%,and the difference was statistically significant(χ^(2)=4.324,4.386,P<0.05).Extubation time(29.6±5.6)min and wake-up time(17.5±6.5)min in the study group were lower than those in the control group(45.6±5.4)min and(26.6±4.8)min,the difference was statistically significant(t=15.391,8.724,P<0.01).Conclusion Comprehensive insulation nursing can effectively reduce the fluctuation of blood glucose index in diabetic patients undergoing general anesthesia,reduce the incidence of hypothermia and chills,and speed up postoperative recovery.It is suitable for clinical application and promotion.
