经脐单孔腹腔镜手术治疗肝胆外科疾病的效果分析

摘　　要：目的研究用经脐单孔腹腔镜手术治疗肝胆外科疾病的可行性和有效性。方法选取在肝胆外科进行治疗的142例患者进行调研,此次调查时间为2018年5月—2019年2月。将患者分为对比组与试验组,每组71例。对比组采用常规三孔腹腔镜手术治疗方式。试验组采用单孔腹腔镜手术治疗方式。分析两组最终治疗结果。结果对比组患者住院时间和试验组相比较长,经过对比发现,差异有统计学意义,t=5.5487、P=0.0000;对比组术中出血量和试验组相比较多、术后排气时间和试验组相比较长、手术时间和试验组相比较长,但两组之间差异无统计学意义,t=1.2037、0.2340、0.6711,P=0.2307、0.8153、0.5033。VAS评分结果两组之间的差异有统计学意义,P<0.05。结论在肝胆外科治疗中,经脐单孔腹腔镜手术可以作为首选的治疗方式,效果优异,值得临床应用。Objective To study the feasibility and effectiveness of transumbilical laparoscopic surgery for hepatobiliary diseases.Methods 142 patients who were treated in hepatobiliary surgery were selected for investigation.The investigation time was from May 2018 to February 2019.Patients were divided into control group and experimental group,71 cases in each group.The control group was treated by conventional three hole laparoscopic surgery.The experimental group was treated by single hole laparoscopic surgery.The final treatment results of the two groups were analyzed.Results Compared with the experimental group,the hospitalization time of the patients in the control group was longer,and the difference was significant(t=5.5487,P=0.0000);compared with the experimental group,the amount of intraoperative bleeding in the control group was more,the time of postoperative exhaust was longer,and the operation time was longer,but there was no significant difference between the two groups,t=1.2037,0.2340,0.6711,P=0.2307,0.8153,0.5033.The difference of VAS score between the two groups was statistically significant(P<0.05).Conclusion In the treatment of hepatobiliary surgery,transumbilical laparoscopic surgery can be used as the first choice,with excellent results,which is worth clinical application.
