从阴阳交理论探讨卒中相关性肺炎

摘　　要：卒中相关性肺炎是由于内伤虚损的机体又复感外邪而发生的温热性疾病,内有正气亏虚、痰瘀留滞,外有六淫之邪侵袭,机体感受外邪后发为肺热病,其临床表现及病理特点与阴阳交理论中的内外合邪发病观类似。治疗卒中相关性肺炎应从患者正气亏虚的实质、病理产物的堆积、感受外邪的性质、感受外邪后机体反应等方面综合考虑,以益气、泻热、养阴为基本原则,兼顾痰瘀等病理因素,方可获效。Stroke-associated pneumonia(SAP)is a kind of warm disease caused by deficiency of internal injury and reinfection of external pathogens.There are deficiency of vital qi,stagnation of phlegm and blood stasis inside,and invasion of six pathogens outside.After the invasion of external pathogens,it develops into pulmonary fever.Its clinical manifestations and pathological characteristics are similar to the theory of internal and external combined pathogens in the theory of yin yang jiao.The treatment of SAP should consider the essence of deficiency of vital qi,the accumulation of pathological products,the nature of external pathogenic factors,and the body reaction after the invasion of external pathogenic factors.The treatment should be based on the basic principles of invigorating qi,relieving heat,nourishing yin,and take into account the pathological factors such as phlegm and blood stasis.Only in this way can it be effective.
