消瘤散癖方防治乳腺增生伴纤维瘤微创术后复发

摘　　要：目的:研究消瘤散癖方对乳腺增生伴纤维瘤患者术后内分泌激素和免疫功能的影响及其可能的机制。方法:采用随机数字表法将90例乳腺增生伴纤维瘤患者分为观察组和对照组,每组各45例。对照组患者采用环乳晕切口微创手术治疗,观察组在采用环乳晕切口微创手术后给予消瘤散癖方。治疗2个月后比较两组患者雌酮(estrone,E_(1))、雌二醇(estradiol,E_(2))、雌三醇(estriol,E_(3))、孕酮(pregesteronum,PT)和雌激素受体(estrogen receptor,ER)表达水平;比较两组患者CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)等T淋巴细胞亚群的表达以及CD_(4)^(+)/CD_(8)^(+)比值,并随访两组患者的复发情况。结果:治疗后,观察组患者E_(1)、E_(2)、E_(3)、PT和ER等内分泌激素水平显著低于对照组,差异有统计学意义(P<0.05);观察组患者CD_(3)^(+)、CD_(4)^(+)等T淋巴细胞亚群指标水平和CD_(4)^(+)/CD_(8)^(+)比值明显高于对照组(P<0.05),CD_(8)^(+)水平低于对照组;观察组复发率4.44%,明显低于对照组的复发率(26.66%)(P<0.05)。结论:乳腺增生伴纤维瘤环乳晕切口手术后,应用消瘤散癖方口服,能够调节患者的内分泌激素水平,提高患者的免疫功能,降低复发率。Objective:To study the effect of Xiaoliu Sanpi Prescription on endocrine hormone and immune function of patients withhyperplastic disease of breast(HDB)with fibroma after operation and its possible mechanism.Methods:90 cases of HDB with fibroma were randomly divided into observation group and control group,45 cases in each group.The control group was treated with minimally invasive surgery through areola incision,and the observation group was treated with Xiaoliu Sanpi Prescription after minimally invasive surgery through areola incision.After two months of treatment,the expression levels of estrone,(E_(1)),estradiol(E_(2)),estriol(E_(3)),progesteronum(PT)and estrogen receptor(ER)were compared between the two groups,and the expression of T lymphocyte subsets such as CD_(3)^(+),CD_(4)^(+),CD_(8)^(+)and CD_(4)^(+)/CD_(8)^(+)ratio were compared,and the recurrence of the two groups were followed up.Results:After treatment,the levels of E_(1),E_(2),E_(3),PT and ER in the observation group were significantly lower than those in the control group(P<0.05);the levels of CD_(3)^(+),CD_(4)^(+)and the ratio of CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher than those in the control group(P<0.05),and the level of CD_(8)^(+)in the observation group was lower than that in the control group;the total recurrence rate in the observation group was 4.44%,which was significantly lower than that in the control group(26.66%)(P<0.05).Conclusion:After the operation of the circular areola incision for HDB with fibroma,Xiaoma powder PI Fang was taken orally,which can regulate the endocrine hormone level of patients,improve the immune function of patients and reduce the recurrence rate.
