我院2020年1~10月阿莫西林钠/氟氯西林钠专项医嘱点评分析

摘　　要：目的评价我院阿莫西林钠/氟氯西林钠使用情况,促进医院抗生素的合理应用。方法通过HIS系统随机抽取2020年1月~10月住院患者使用阿莫西林钠/氟氯西林钠病例进行医嘱点评,总共100份。采取回顾性分析药物利用评价(DUE)方法评价,并对抗菌药物阿莫西林钠/氟氯西林钠的使用情况及存在的问题进行分析。结果随机抽查100份病例,使用阿莫西林钠/氟氯西林钠概率较高的科室分别为消化肾内分泌科,占比33.00%;骨科,占比28.00%;普外科,占比17.00%。通过HIS系统对医嘱进行专项点评,不合理率为10%,不合理占比较高的前三个科室分别为呼吸科(33.33%)、消化肾内分泌(12.12%)、普外科(11.76%)。不合理用药情况主要类型有:无使用指征、药物遴选不合理、用法用量不合理、不合理联用。结论我院阿莫西林钠/氟氯西林钠不合理使用情况较严重,需要采取抽查点评并持续干预改进等措施,促进临床合理使用阿莫西林钠/氟氯西林钠的水平,保障患者用药合理、安全、有效。Objective To evaluate the use of amoxicillin sodium and flucloxacillin sodium in our hospital and promote the rational use of antibiotics in hospital.Methods A total of 100 cases of amoxicillin sodium flucloxacillin sodium were randomly selected from hospitalized patients from January to October 2020 by HIS system for medical order review.Retrospective analysis of drug utilization evaluation(DUE)method was used to evaluate,and the use of the antibacterial drug amoxicillin sodium flucloxacillin sodium and the existing problems were analyzed.Results Randomly check 100 cases.The departments with higher probability of using amoxicillin sodium/flucloxacillin sodium are digestion,kidney and endocrinology,accounting for 33.00%;orthopedics,accounting for 28.00%;general surgery,accounting for 17.00%in the 100 cases checked randomly.The unreasonable rate was10%in the special comments on doctor orders through the HIS system,and the top three departments with a relatively high proportion of unreasonable were respiratory department(33.33%),digestion and renal endocrinology(12.12%),and general surgery(11.76%).The main types of irrational drug use are:no indication for use,unreasonable drug selection,unreasonable usage and dosage,and unreasonable combination.Conclusion The irrational use of amoxicillin sodium and flucloxacillin sodium in our hospital is serious,and it is necessary to take measures such as spot check and comment and continuous to promote the rational use of amoxicillin sodium and flucloxacillin sodium in clinical practice and ensure the rational,safe and effective use of drugs by patients.
