血清变应原特异性IgE和外周血嗜酸粒细胞在霉菌性鼻窦炎中的表达及临床意义

摘　　要：目的:探讨血清变应原特异性IgE(sIgE)和外周血嗜酸粒细胞(EOS)在霉菌性鼻窦炎中的表达及临床意义。方法:选取60例霉菌性鼻窦炎患者设为鼻窦炎组,同期疑似霉菌性鼻窦炎但经金标准排除诊断的60例患者作为对照组。检测所有患者血清变应原sIgE、外周血EOS,分析两者之间的相关性;采取受试者工作特征曲线(ROC)及曲线下面积(AUC)分析两者诊断鼻窦炎的价值。结果:鼻窦炎组血清变应原sIgE阳性率、外周血EOS阳性率均高于对照组(P<0.05),血清变应原sIgE水平、外周血EOS计数均高于对照组(P<0.05)。Pearson相关性分析显示鼻窦炎患者血清变应原sIgE与外周血EOS计数无相关性(P>0.05)。ROC曲线分析显示,血清变应原sIgE诊断鼻窦炎的AUC值为0.779,截断值为1.94 IU/L,敏感度为68.33%,特异度为78.33%;外周血EOS计数诊断鼻窦炎的AUC值为0.752,截断值为0.55×10^(9)/L,敏感度为61.67%,特异度为81.67%;两者联合检测诊断鼻窦炎的AUC为0.846,高于单一检测(P<0.05)。结论:血清sIgE与外周血EOS对霉菌性鼻窦炎有较高的诊断价值,两者联合检测可提高诊断效能。Objective:To investigate the clinical significance of serum allergen-specific IgE(sIgE)and peripheral blood eosinophils(EOS)in the expression of fungal sinusitis.Methods:60 patients with fungal sinusitis were selected as the sinusitis group,and 60 patients with suspected fungal sinusitis but excluded from the diagnosis by the gold standard were served as control group.Serum allergen sIgE and peripheral blood EOS were detected among all the patients,and the correlation between the two was analyzed.The receiver operating characteristic curve(ROC)and the area under the curve(AUC)were used to analyze the value of the two in the diagnosis of sinusitis.Results:The positive rate of serum allergen sIgE and positive rate of peripheral blood EOS in sinusitis group were significantly higher than those in control group(P<0.05),and the serum allergen sIgE level and peripheral blood EOS count were higher than those in control group(P<0.05).Pearson correlation analysis showed that there was no significant correlation between serum allergen sIgE and peripheral blood EOS count in patients with sinusitis(P>0.05).ROC curve analysis showed that the AUC value,cutoff value,sensitivity and specificity of serum allergen sIgE in the diagnosis of sinusitis were 0.779,1.94 IU/L,68.33%and 78.33%.The AUC value,cutoff value,sensitivity and specificity of peripheral blood EOS count in the diagnosis of sinusitis were 0.752,0.55×10^(9)/L,61.67%and 81.67%.The AUC of the combined detection of sinusitis was 0.846,which was significantly higher than that of single detection(P<0.05).Conclusion:Serum sIgE and peripheral blood EOS have a high diagnostic value on fungal sinusitis,and combined detection of the two can improve diagnostic efficacy.
