过伸体位下椎体后凸成形术治疗Kummell病的临床疗效分析

摘　　要：目的:探讨过伸体位下椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗Kummell病的临床疗效及其并发症情况。方法:选取2013年1月~2017年1月收治的Kummell病患者38例,采用PKP治疗,测量术前、术后2天和术后6个月椎体前缘高度、中央高度及Cobb角,并采用视觉模拟量表(visual analog scale,VAS)评分,采用Oswestry功能障碍指数(Oswestry disability index,ODI)评估手术治疗效果。结果:所有患者均进行随访,术后6个月VAS评分(2.6±0.7)分明显低于术前VAS评分(7.4±1.2)分,差异有统计学意义(P<0.01);术后6个月(21.5±8.1)分明显低于术前ODI(85.6±7.8)分,差异有统计学意义(P<0.01);术后6个月椎体前缘高度(23.5±2.1)mm、中央高度(26.6±2.1)mm分别明显高于术前(16.3±2.3)mm、(19.4±2.2)mm,差异有统计学意义(P<0.01);术后6个月Cobb角(13.4±3.7)°明显低于术前(22.4±3.2)°,差异有统计学意义(P<0.01)。结论:过伸体位下PKP对于无神经症状的Kummell病是一种有效的治疗办法,能够明显缓解疼痛,稳定椎体,部分恢复椎体的高度及矫正畸形,并且不增加骨水泥渗漏及邻近椎体再骨折风险。Objective To investigate the clinical efficacy and complications of hypercutaneous kyphoplasty(PKP)in the treatment of Kummell disease.Method Thirty-eight patients with Kummell disease treated in our hospital from January 2013 to January 2017 were treated with PKP.The anterior edge height,central height and Cobb angle of the vertebral body were measured before,2 days and 6 months after operation.The visual analog scale(VAS)score and the Oswestry disability index(ODI)were used to evaluate the effect of surgical treatment.Results All patients were followed up.The VAS score(2.6±0.7)at 6 months after operation was significantly lower than the preoperative VAS score(7.4±1.2)(P<0.01);The ODI score(21.5±8.1)at 6 months after surgery was significantly lower than the preoperative ODI score(85.6±7.8)(P<0.01).The anterior edge height of the vertebra at 6 months after surgery(23.5±2.1)mm and central height(26.6±2.1)mm were significantly higher than(16.3±2.3)mm and(19.4±2.2)mm before surgery(P<0.01);The Cobb angle(13.4±3.7)°at 6 months after operation was significantly lower than that before operation(22.4±3.2)°(P<0.01).Conclusion PKP is an effective treatment for Kummell′s disease without neurological symptoms,which can significantly relieve pain,stabilize the vertebral body,partially restore the height of the vertebral body and correct deformities,and does not increase bone cement leakage and proximity Risk of vertebral fractures.
