摘 要:目的:观察2型糖尿病并发心力衰竭患者尿液钙整合素结合蛋白1(CIB 1)及血清B型脑利钠肽前体(pro-BNP)水平及其与心功能分级、心功能指标相关性,分析影响2型糖尿病并发心力衰竭的危险因素。方法:85例2型糖尿病并发心力衰竭患者为观察组,另选取心功能正常的2型糖尿病患者85例为对照组。检测两组患者尿CIB 1和血清pro-BNP水平,心脏彩色多普勒超声检测LAD、RAD、LVESD、LVEDD及LVEF。比较两组尿CIB 1及血清pro-BNP水平及其与心功能分级、心功能指标的相关性,采用Pearson法进行相关性分析,Logistic回归分析影响2型糖尿病并发心力衰竭的危险因素。结果:观察组尿CIB 1为3.59±0.17μg/L,血清pro-BNP为556.28±123.57 ng/L,分别高于对照组的1.25±0.13μg/L和55.67±12.64 ng/L,差异均具有统计学意义(P<0.05)。观察组患者尿CIB 1及血清pro-BNP水平随着心功能分级增加而增高,差异均具有统计学意义(P<0.05)。观察组尿CIB 1、血清pro-BNP水平与LAD、RAD、LVESD及LVEDD呈正相关,与LVEF呈显著负相关,均具有统计学意义(P<0.05)。Pearson相关性分析显示尿CIB 1与血清pro-BNP水平正相关,具有统计学意义(P<0.05)。Logistic回归分析显示糖尿病病程、空腹血糖、血清pro-BNP、尿CIB 1水平均为影响2型糖尿病并发心力衰竭的危险因素。结论:2型糖尿病并发心力衰竭患者尿CIB 1、血清pro-BNP水平升高,二者呈正相关,并与心功能分级及心功能指标密切相关,联合检测尿液CIB1和血清pro-BNP可作为2型糖尿病并发心力衰竭诊断及病情预测的重要生物指标。Objective:To observe the level of urine recombinant calcium and integrin binding protein 1(CIB1)and serum pro-B-type brain natriureticin peptide(pro-BNP)in patients with type 2 diabetes mellitus complicated with heart failure and the correlation with cardiac function grade and index,and to analyze the risk factors of type 2 diabetes mellitus complicated with heart failure.Methods:85 patients with type 2 diabetes mellitus complicated with heart failure were selected as the observation group and 85 patients with normal heart function as the control group.The levels of urine CIB 1 and serum pro-BNP were measured,LAD,RAD,LVESD,LVEDD and LVEF were detected by color Doppler echocardiography.The levels of urine CIB 1 and serum pro-BNP were compared between the two groups,their correlation with cardiac function grade and index was analyzed by Pearson method,and the risk factors of type 2 diabetes mellitus complicated with heart failure were analyzed by Logistic regression.Results:The urine CIB 1 was 3.59±0.17μg/L in the observation group,and the serum pro-BNP was 556.28±123.57 ng/L,which was higher than that of the control group 1.25±0.13μg/L and 55.67±12.64 ng/L respectively,the difference was statistically significant(P<0.05).The level of urine CIB 1 and serum pro-BNP in the observation group increased with the increase of cardiac function grade,and the difference was statistically significant(P<0.05).The levels of CIB 1 in urine and pro-BNP in serum were positively correlated with LAD,RAD,LVESD and LVEDD(P<0.05),and negatively correlated with LVEF,all of which had statistical significance(P<0.05).Pearson correlation analysis showed that urine CIB 1 was positively correlated with serum pro-BNP level,with statistical significance(P<0.05).Logistic regression analysis showed that the course of diabetes,fasting blood glucose,pro-BNP and CIB 1 were all risk factors for type 2 diabetes mellitus complicated with heart failure.Conclusion:The level of urine CIB 1 and serum pro-BNP in patients with type 2 diabetes mellitus
