摘 要:目的探讨线上复诊平台在辅助生殖治疗中的应用价值。方法选取2020年3月至6月于中国医科大学附属盛京医院生殖医学中心进行体外受精-胚胎移植(IVF-ET)治疗的患者,按照复诊方式分为传统组和线上组。通过调查问卷方式评估线上复诊平台的使用情况,分析患者基本特征与复诊方式选择的关联,比较2组的医疗费用以外成本(包括经济和时间成本)及满意度。结果93%的患者认为线上复诊平台提供了很好的医疗服务。患者的学历(OR=1.248,95%CI:1.002~1.554)、周期次数(OR=1.715,95%CI:1.052~2.795)、住址到医院的往返费用(OR=1.001,95%CI:1.000~1.002)、往返距离(OR=1.005,95%CI:1.004~1.006)、往返时间(OR=1.507,95%CI:1.333~1.705)是患者选择线上复诊方式的风险因素。首次就诊到开始促排卵期间,线上组患者的返院次数[3(2~4)]少于传统组[4(3~7)],平均节约了经济成本859.91元和时间成本10.10 h。使用线上复诊平台缩短了首次就诊到开始促排卵的时间间隔。线上组患者医疗满意度高于传统组。结论线上复诊平台有效减少了患者复诊次数,节约了医疗费用以外成本,提高了满意度。Objective To assess the application value of an online consultation platform in assisted reproductive techniques.Methods Two available options(traditional consultation and online consultation)were provided to in vitro fertilization and embryo transfer(IVF-ET)patients involving a series of routine examinations requested before controlled ovarian hyperstimulation(COH).The effect of the online consultation platform was analyzed through a questionnaire.Additionally,we analyzed the relevance between patients’characteristics and the consultation mode selection,and compared the costs additional to medical expenses as well as patient satisfaction between the two groups.Results Ninety-three percent of patients believed that the online consultation platform provided good medical services.Risk factors for selection of online consultation mode included education level(OR=1.248,95%CI:1.002-1.554),IVF cycle times(OR=1.715,95%CI:1.052-2.795),expenses(OR=1.001,95%CI:1.000-1.002),distance between the home and hospital(OR=1.005,95%CI:1.004-1.006),and time(OR=1.507,95%CI:1.333-1.705).During the period from the first visit to the initiation of COH,the frequency of returning to the hospital of the online group[3(2-4)]was significantly lower than that of the traditional group[4(3-7)],with mean economic cost saving of CNY 859.91 and mean time saving of 10.10 hours.The online consultation platform can shorten the time interval between the first visit to the initiation of COH.The satisfaction of patients in the online group was notably higher than in the traditional group.Conclusion The online consultation platform effectively reduces patients’return visit frequency and costs additional to medical expenses,consequently enhancing patient satisfaction.
