摘　　要：目的:探究补肾化痰方治疗肥胖型多囊卵巢综合征(PCOS)疗效及对患者胰岛素抵抗指数(HOMA-IR)、腰臀比(WHR)和载脂蛋白影响。方法:选取2016年12月—2018年7月本院收治的肥胖型PCOS患者110例,随机分为西药组和中西药组各55例,西药组给予常规口服炔雌醇环丙孕酮片治疗,中西药组在西药组方法基础上加用补肾化痰方治疗,分析两组临床疗效。结果:治疗后中西药组载脂蛋白-A(1.77±0.56 g/L)、血清脂联素(ADPN)(10.63±2.12 mg/L)水平高于西药组,HOMA-IR(1.58±0.65)、体质指数(24.25±3.10 kg/m2)、WHR(1.14±0.22)、载脂蛋白-B(0.45±0.12 g/L)水平低于西药组,毛发评分(5.10±1.98分)、痤疮评分(0.31±0.12分)、月经失调评分(8.79±1.89分)低于西药组,妊娠率(60.0%)高于西药组(40.0%)(均P<0.05),排卵率两组无差异(P>0.05)。结论:补肾化痰方可改善肥胖型PCOS患者的临床症状,调节血脂,降低HOMA-IR、BMI、WHR水平,提高妊娠率。Objective:To explore the effect of Bushen huatan recipe for treating obese women with polycystic ovary syndrome(PCOS),and to study its influence on the insulin resistance index(HOMA-IR),waist-to-hip ratio(WHR)and apolipoprotein level of these women.Methods:110 obese women with PCOS were selected and were divided into two groups from December 2016 to July 2018(55 cases in each group).The women in the control group were treated with routine ethinylestradiol and cyproterone pills,and the women in the observation group were treated with routine ethinylestradiol and cyproterone pills combined with Bushen huatan recipe.The clinical efficacy of the women in the two groups were analyzed.Results:After treatment,the levels of apolipoprotein-A(1.77±0.56 g/L)and serum adiponectin level(10.63±2.12 mg/L)of women in the observation group were significant higher than those of women in the control group,but the level of HOMA-IR(1.58±0.65),BMI value(24.25±3.10 kg/m2),WHR value(1.14±0.22),and apolipoprotein-B level(0.45±0.12 g/L)of women in the observation group were significant lower than those of women in the control group.The hair score(5.10±1.98 points),acne score(0.31±0.12 points),menstrual disorder score(8.79±1.89 points)of women in the observation group were significant lower than those of women in the control group,but the pregnancy rate(60.0%)of women in the observation group was significant higher than that(40.0%)of women in the control group(all P>0.05).There was no significant difference in the ovulation rate of women between the two groups(P>0.05).Conclusion:Bushen huatan recipe can alleviate the clinical symptoms of obese women with PCOS,regulate their blood lipid levels,reduce their values of HOMA-IR,BMI,and WHR,and increase their pregnancy rate.
