肝瞬时弹性成像与天冬氨酸转氨酶/血小板指数在慢性乙型肝炎合并脂肪肝患者肝纤维化诊断中的应用价值

摘　　要：目的探讨肝瞬时弹性成像(Fibroscan)与天冬氨酸转氨酶/血小板指数(APRI)在慢性乙型肝炎(CHB)合并脂肪肝患者肝纤维化诊断中的应用价值。方法选取阳江市公共卫生医院2017年1~12月收治的CHB合并脂肪肝患者,选择其中的46例展开回顾性分析,患者均实施Fibroscan及APRI检测。对患者追踪随访2年,观察该阶段患者的纤维化情况,进行病情监控。结果Fibroscan与APRI联合检测在纤维化不同程度诊断检出率方面与病理检测方面比较,差异无统计学意义(P>0.05)。治疗2年后的APRI、Fibroscan、AST指标均低于初始值,差异有统计学意义(P<0.05);而血小板(PLT)指标与初始值比较,差异无统计学意义(P>0.05)。随着肝纤维化严重程度的加重,APRI、Fibroscan检测随之升高。结论CHB合并脂肪肝患者使用Fibroscan与APRI联合的方式可以对肝纤维化的变化情况进行综合分析,并且对疾病进行动态化监控,其对于疾病的监控起到积极意义,可提升疾病的综合干预质量,利于后期疾病的治疗。Objective To investigate the value of Fibroscan and aspartate aminotransferase(AST)to platelet ratio index(APRI)in the diagnosis of liver fibrosis in patients with chronic hepatitis B(CHB)complicated with fatty liver.Methods CHB patients with fatty liver from January to December 2017 in Yangjiang Public Health Hospital were selected,and 46 cases were selected for retrospective analysis.The patients were all performed Fibroscan and APRI detection.The patients were followed up for 2 years to observe the fibrosis of the patients at this stage and to monitor their condition.Results There was no significant difference between Fibroscan and APRI in the detection rate of different degrees of fibrosis and pathological detection(P>0.05).After 2 years of treatment,the APRI,Fibroscan,AST indexes were lower than the initial value,the differences were statistically significant(P<0.05).While there was no significant difference in the blood platelet indexes between after 2 years of treatment and initial value(P>0.05).With the aggravation of liver fibrosis,the detection of APRI and Fibroscan increased.Conclusion The combination of Fibroscan and APRI in CHB patients with fatty liver can comprehensively analyze the changes in liver fibrosis,and dynamically monitor the disease,which has a positive significance for disease monitoring and can improve the quality of comprehensive interventions,is conducive to the treatment of later disease.
