摘 要:目的探讨外阴阴道念珠菌病(vulvovaginal candidiasis,VVC)患者血清及阴道分泌物中超氧化物歧化酶(superoxide dismutase,SOD)、谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)、氧化末端产物晚期糖基化终末产物(advanced glycation end products,AGEs)、晚期氧化蛋白产物(advanced oxidation protein products,AOPP)的表达水平及临床意义。方法选本院就诊的51例轻中度、21例重度VVC及33例复发性外阴阴道念珠菌病(recurrent vulvovaginal candidiasis,RVVC)患者为观察组,39例正常体检者为对照组,采用ELISA检测SOD、GSH-Px、AGE、AOPP水平,分析各指标及其与VVC临床症状间的相关性。结果血清中,重度VVC及RVVC组AGEs水平低于对照组(P均<0.05),轻中度VVC及重度VVC组AOPP水平均低于对照组(P均﹤0.05),重度VVC组SOD及GSH-Px水平高于对照组(P均﹤0.05);分泌物中,重度VVC组AGEs、AOPP水平显著低于对照组(P均﹤0.05),轻中度VVC、重度VVC组、RVVC组SOD水平均高于对照组(P均﹤0.05),重度VVC组、RVVC组GSH-Px水平均高于对照组(P均﹤0.05)。相关性分析发现:分泌物中AGEs、AOPP与SOD、GSH-Px表达均呈负相关(r=-0.25,-0.45;P均﹤0.05);血清中GSH-Px与疾病严重程度呈正相关(r=0.21,P﹤0.05),分泌物中SOD与疾病严重程度呈正相关(r=0.22,P﹤0.05)。结论VVC患者体内存在AGE、AOPP水平下调及SOD、GSH-Px水平上调,氧化应激相关因子SOD、GSH-Px水平与VVC的疾病严重程度有关,SOD、GSH-Px、AGE、AOPP可能参与VVC的发病。Objective To investigate the expression and clinical significance of superoxide dismutase(SOD),glutathione peroxidase(GSH-Px),advanced glycation end products(AGEs),advanced oxidation protein products(AOPP)in serum and excretion of patients with vulvovaginal candidiasis(VVC).Methods This study included 51 patients with mild-to-moderate VVC,21 with severe VVC,and 33 with recurrent vulvovaginal candidiasis(RVVC)as the observation group,and 39 healthy people who underwent physical examination as the controls.ELISA was used to detect the expression of SOD,GSH-Px,AGE and AOPP,and the association of the expression of these indexes with the clinical symptoms of VVC was analyzed.Results In serum,the levels of AGEs in severe VVC group and RVVC group were lower than those in control group(both P<0.05).The levels of AOPP in mild-to-moderate VVC and severe VVC groups were higher than those in control group(both P<0.05),and the levels of SOD and GSH-Px in severe VVC group were higher than those in the control group(both P<0.05).In the secretion,the levels AGEs and AOPP in the severe VVC group were significantly lower than those in control group(both P<0.05).The levels of SOD in mild-to-moderate VVC,severe VVC and RVVC groups were significantly higher than those in control group(all P<0.05).And GSH-Px in the severe VVC and RVVC groups were significantly higher than those in control group(both P<0.05).Correlation analysis found that in secretion,AGEs and AOPP in secretion were negatively correlated with SOD and GSH-Px expression(r=-0.25,-0.45;P<0.05).GSH-Px in serum was positively correlated with the severity of the disease(r=0.21,P<0.05),and SOD in secretion was positively correlated with disease severity(r=0.22,P<0.05).Conclusion The down-regulation of AGE,AOPP and upregulation of SOD and GSH-Px were detected in patients with VVC.The oxidative stress-related factor SOD and GSH-Px were correlated with the severity of the disease.These laboratory indicators may be involved in the pathogenesis of VVC.
