细菌性包皮龟头炎50例临床分析

摘　　要：目的了解细菌性包皮龟头炎的致病菌种及其临床特征,为临床医生对该病的正确识别提供帮助。方法对包皮龟头炎患者详细询问病史、记录临床症状、体征,并做病原学检查。结果50例细菌培养阳性包皮龟头炎患者,共培养出53株,13种细菌,检出率最高的前四位菌种依次是表皮葡萄球菌15株(28.30%)、无乳链球菌8株(15.09%)、化脓性链球菌6株(11.32%)、大肠埃希菌6株(11.32%)。表皮葡萄球菌感染通常临床症状轻微,病程慢性,常表现为包皮、龟头红斑,干燥、脱屑或包皮垢增多。无乳链球菌、化脓性链球菌及大肠埃希菌感染通常临床症状较重,呈急性病变。无乳链球菌及化脓性链球菌感染常表现为包皮、龟头弥漫性红斑、糜烂、渗出;大肠埃希菌感染通常表现为包皮、龟头弥漫性红斑、丘疹、糜烂或密集乳酪样斑点、丘疹、糜烂。结论无乳链球菌、化脓性链球菌及大肠埃希菌为引起包皮龟头急性病变的主要致病菌种;表皮葡萄球菌为引起包皮、龟头慢性病变的主要致病菌种。Objective To understand the pathogenic bacteria species and clinical characteristics of balanoposthitis caused by bacterial infection,and to offer assistance for clinicians to make diagnosis.Methods Detailed medical history of patients with balanoposthitis was taken,clinical symptoms and signs were recorded,and pathogenic examination was performed.Results A total of 53 strains and 13 species were identified among 50 patients with positive bacterial culture,and the most common four pathogenic bacteria strains were Staphylococcus epidermidis(15 strains,28.30%),Streptococcus agalactiae(8 strains,15.09%),Streptococcus pyogenes(6 strains,11.32%)and Escherichia coli(6 strains,11.32%).The clinical symptoms of S.epidermidis infection were usually mild,often manifested as foreskin,erythema,dryness,desquamation of foreskin,glans,or increased foreskin scale,and the course of disease is usually chronic.The infection caused by S.agalactiae,S.pyogenes,and E.coli were usually more severe,presenting acute lesions.The clinical symptoms of S.agalactiae and S.pyogenes infection were often manifested as foreskin,diffuse erythema,erosion,exudation of foreskin,glans.E.coli infection was usually manifested as foreskin,erythema,erosion or multiple cheese like spots,papules and erosions.Conclusion S.agalactiae,S.pyogenes and E.coli are the main pathogenic bacteria that cause acute balanoposthitis.S.epidermidis is the main pathogenic bacteria that cause chronic balanoposthitis.
