64-MSCT检查对脂肪肝合并肝脏结节性病变的诊断价值

摘　　要：目的探讨64层螺旋CT(64-MSCT)检查对脂肪肝合并肝脏结节性病变患者诊断准确率的影响。方法选取2019年2月—2020年4月新乡市中心医院收治的脂肪肝合并肝脏结节性病变患者120例作为研究对象,均接受病理及64-MSCT检查,并将病理学结果作为金标准,分析64-MSCT在脂肪肝合并肝脏结节性病变中的诊断效能(敏感度、特异度、漏诊率、误诊率、准确率)。结果120例脂肪肝合并肝脏结节性病变患者,病理学检出结节145个,其中81例良性患者检出良性结节98个,占67.59%(98/145),39例恶性患者检出恶性结节47个,占32.41%(47/145);120例脂肪肝合并肝脏结节性病变患者中,64-MSCT检出恶性结节44个,检出良性结节89个;64-MSCT检测脂肪肝合并肝脏结节性病变敏感度为93.62%(44/47)、特异度90.82%(89/98)、漏诊率6.38%(3/47)、误诊率9.18%(9/98)、准确率91.72%(133/145)。结论在脂肪肝合并肝脏结节性病变患者诊断中,64-MSCT检查能提高脂肪肝合并肝脏结节性病变诊断准确率,减少误诊和漏诊,可为临床制定治疗方案提供影像学依据。Objective To investigate the influence of 64-slice spiral CT(64-MSCT)examination on the diagnostic accuracy of patients with fatty liver complicated with hepatic nodular degeneration.Methods From February 2019 to April 2020 patients with fatty liver combined liver nodular lesions of our hospital 120 cases as the research object,were examined by pathology and 64-MSCT,results and pathology as the gold standard,the analysis of 64-MSCT in the diagnosis of fatty liver combined liver nodular lesions efficiency(sensitivity,specific degree,the missed diagnosis and misdiagnosis rate and accuracy).Results In 120 patients with fatty liver complicated with hepatic nodular degeneration,145 nodules were detected pathologically,among which 98 nodules were detected in 81 benign patients(67.59%(98/145)and 47 nodules were detected in 39 malignant patients(32.41%(47/145).Among 120 patients with fatty liver complicated with hepatic nodular degeneration,44 malignant nodules and 89 benign ones were detected by 64-MSCT.The sensitivity of 64-MSCT to detect fatty liver with nodular liver lesions was 93.62%(44/47),specificity 90.82%(89/98),missed diagnosis rate 6.38%(3/47),misdiagnosis rate 9.18%(9/98),and accuracy rate 91.72%(133/145).Conclusion in the diagnosis of fatty liver with hepatic nodular lesions,64-MSCT examination can improve the diagnostic accuracy of fatty liver with hepatic nodular lesions,reduce misdiagnosis and missed diagnosis,and provide imaging basis for clinical treatment.
