咪达唑仑对异丙酚-瑞芬太尼全凭静脉麻醉下脊柱矫形术患者术中唤醒试验效果的影响

摘　　要：目的:探讨脊柱矫形术异丙酚-瑞芬太尼全凭静脉麻醉患者咪达唑仑对术中唤醒试验效果的影响.方法:回顾性选取2018年11月-2019年11月我院脊柱矫形术患者100例,依据麻醉方法分为异丙酚-瑞芬太尼全凭静脉麻醉组(对照组,n=50)和异丙酚-瑞芬太尼全凭静脉麻醉联合咪达唑仑组(观察组,n=50)2组,统计分析2组患者的血流动力学、麻醉诱导到开始唤醒试验时间、麻醉时间、手术时间、唤醒时间、不良反应发生情况.结果:2组患者T2时的心率、平均动脉压、听觉诱发电位指数均显著高于T1、T3时(P<0.05).观察组患者的麻醉诱导到开始唤醒试验时间、麻醉时间、手术时间均显著短于对照组(P<0.05),唤醒时间显著长于对照组(P<0.05).在不良反应发生率方面,观察组为12.0%(6/50),对照组为50.0%(25/50),前者显著低于后者(P<0.05).结论:脊柱矫形术异丙酚-瑞芬太尼全凭静脉麻醉患者咪达唑仑能够有效改善术中唤醒试验效果.Objective:To investigate the effect of midazolam on intraoperative wake-up test in patients undergoing total intravenous anesthesia with propofol remifentanil.Methods:100 cases of patients with spinal orthopedics in our hospital from November 2018 to November 2019 were selected retrospectively.According to the anesthesia methods,they were divided into two groups:Propofol remifentanil total intravenous anesthesia group(control group,n=50)and propofol remifentanil total intravenous anesthesia combined with midazolam group(study group,n=50).The hemodynamics,time from anesthesia induction to start of wake-up test,anesthesia tim e,operation time,wake-up time and adverse reactions of the two groups were statistically analyzed.Results:The heart rate,mean arterial pressure and auditory evoked potential index of the two groups at T2 were significantly higher than at T1 and T3(P<0.05).The time from anesthesia induction to the start of wake-up test,anesthesia time and operation time of the study group were significantly shorter(P<0.05),the wake-up time was significantly longer than the control group(P<0.05).The incidence of adverse reactions was 12.0%(6/50)in the study group and 50.0%(25/50)in the control group.The former was significantly lower than the latter(P<0.05).Conclusion:Midazolam can effectively improve the effect of wake-up test of patients undergoing total intravenous anesthesia with propofol remifentanil.
