摘 要:目的探讨平滑肌蛋白抗体(Smoothelin)、组织蛋白酶D(CAD)、S-100蛋白在先天性巨结肠患儿肠道组织内的表达及意义。方法选取在2015年3月至2019年3月期间甘肃省妇幼保健院收治的先天性巨结肠患儿132例,分别取患儿术中切除的病变肠段的狭窄段、移行段及扩张段。比较各病变肠断的Smoothelin RNA和蛋白的表达、CAD、S-100蛋白阳性表达及肌间神经丛、成熟神经节细胞数量。独立样本t检验比较其差异性。结果患儿扩张段的Smoothelin mRNA(27.15±16.46)及蛋白灰度值(43.13±12.46)显著高于狭窄段(21.62±10.03、36.82±9.44)及移行段(19.64±9.91、23.65±4.96,t=4.491、3.296、16.688、4.638,P值均<0.05),且移行段的Smoothelin蛋白灰度值显著高于狭窄段,差异有统计学意义(t=14.189,P<0.05)。患儿扩张段的CAD阳性表达的占比(82.58%)显著高于移行段、狭窄段(31.82%、0.00%,χ^(2)=185.652、69.454,P值均<0.05),且移行段CAD阳性表达的占比显著高于狭窄段(χ^(2)=49.946,P<0.05),扩张段S-100蛋白阳性表达的占比(29.55%)显著低于移行段、狭窄段(51.52%、90.15%,χ^(2)=100.884、13.217,P值均<0.05),且移行段S-100蛋白阳性表达的占比显著低于狭窄段,差异有统计学意义(χ^(2)=47.688,P<0.05)。患儿扩张段的肌间神经丛[(3.80±0.54)个]及成熟神经节细胞数量[(3.12±0.22)个]显著高于移行段[(3.12±0.37)、(2.41±0.38)个]、狭窄段[(2.13±0.32)、(0±0)个,t=11.935、30.566、18.578、162.937,P值均<0.05],且移行段的肌间神经丛及成熟神经节细胞数量显著高于狭窄段,差异有统计学意义(t=23.251、72.865,P<0.05)。结论Smoothelin、CAD、S-100蛋白的表达与先天性巨结肠的发生发展密切相关,其发病机制可能与神经节及神经纤维细胞数量有关。Objective To investigate the expression and significance of smoothelin,cathepsin D(CAD),and S-100 in the intestinal tissue of children with congenital megacolon.Methods From March 2015 to March 2019,132 cases of congenital megacolon were retrospectively analyzed.The narrow segment,transitional segment and dilated segment of the diseased intestinal segment were removed in the children.The positive expression of Smoothelin,CAD,S-100 and the number of muscle plexus and mature ganglion cells in each lesion were compared.The χ^(2) test or independent sample t test was used to compare their differences.Results The Smoothelin mRNA(27.15±16.46)and gray value of protein(43.13±12.46)in the dilated segment were significantly higher than those in the narrow segment(21.62±10.03,36.82±9.44)and transitional segment(19.64±9.91,23.65±4.96,t=4.491,3.296,16.688,4.638,allP<0.05),and the gray value of Smoothelin protein in the transitional segment was significantly higher than that in the narrow segment with the difference being statistically significant(t=14.189,P<0.05).The CAD positive expression(82.58%)was significantly higher in the dilated segment than in the transitional and narrow segments(31.82%,0.00%,χ^(2)=185.652,69.454,allP<0.05).The positive expression rate of CAD in the transitional segment was significantly higher than that in the narrow segment(χ^(2)=49.946,P<0.05).The positive expression rate of S-100 protein in expansion segment(29.55%)was significantly lower than that in transitional segment and narrow segment(51.52%,90.15%,χ^(2)=100.884,13.217,P all<0.05).The positive expression rate of S-100 protein in transitional segment was significantly lower than that in narrow segment with the difference being statistically significant(χ^(2)=47.688,P<0.05).The number of muscle plexus[(3.80±0.54)cells]and mature ganglion cells[(3.12±0.22)cells]in the dilated segment was significantly greater than that in the transitional segment[(3.12±0.37),(2.41±0.38)cells],the stenosis segment[(2.13±0.32),(0±0)cells,t=11.93
