抗凝及导管溶栓联合下腔静脉滤器置入治疗下肢深静脉血栓的疗效

摘　　要：目的研究在下肢深静脉血栓中应用抗凝及导管溶栓与下腔静脉滤器置入联合治疗的临床效果。方法 80例下肢深静脉血栓患者,采用随机信封分组法分为对照组和实验组,每组40例。对照组采用抗凝和系统溶栓治疗,实验组在抗凝和导管溶栓治疗基础上联合下腔静脉滤器置入治疗。比较两组的治疗效果、治疗前后大腿周径差和小腿周径差及并发症发生率。结果实验组治疗总有效率97.50%高于对照组的85.00%,差异具有统计学意义(P<0.05)。治疗后,两组大腿周径差、小腿周径差均较治疗前下降,且实验组大腿周径差(1.22±1.20)cm、小腿周径差(1.17±1.10)cm均低于对照组的(1.98±1.27)、(1.87±1.17)cm,差异具有统计学意义(t=2.751、2.757, P=0.007、0.007<0.05)。实验组并发症发生率5.00%低于对照组的20.00%,差异具有统计学意义(χ^(2)=4.114, P=0.043<0.05)。结论在下肢深静脉血栓中应用抗凝及导管溶栓与下腔静脉滤器置入联合治疗,能有效降低大腿、小腿周径差,降低并发症发生率,对转归改善、治疗总有效率提升具有明显促进作用,值得各大医院推广和使用。Objective To study the clinical effect of anticoagulation and catheter-directed thrombolysis combined with inferior vena cava filter placement in the treatment of lower extremity deep venous thrombosis.Methods A total of 80 patients with lower extremity deep venous thrombosis were divided into control group and experimental group according to random envelope grouping method,with 40 cases in each group.The control group was treated with anticoagulation and systemic thrombolysis,and the experimental group was treated with anticoagulation and catheter-directed thrombolysis combined with inferior vena cava filter placement.The therapeutic effect,difference of thigh circumference and calf circumference before and after treatment,and occurrence of complications were compared between the two groups.Results The total effective rate 97.50%of the experimental group was higher than 85.00%of the control group,and the difference was statistically significant(P<0.05).After treatment,the difference of thigh circumference and calf circumference of the two groups were decreased,and thigh circumference(1.22±1.20)cm and calf circumference(1.17±1.10)cm of the experimental group were lower than(1.98±1.27)and(1.87±1.17)cm of the control group,and the difference was statistically significant(t=2.751,2.757;P=0.007,0.007<0.05).The incidence of complications 5.00%of the experimental group was lower than 20.00%of the control group,and the difference was statistically significant(x^(2)=4.114,P=0.043<0.05).Conclusion For the treatment of lower extremity deep venous thrombosis,anticoagulation and catheter-directed thrombolysis combined with inferior vena cava filter placement can effectively reduce the difference in the circumference of the thigh and calf,lower the incidence of complications.It has a significant role in improving the outcome and total effective rate of treatment,and is worthy of promotion and application.
