乳管镜灌洗液液基细胞学检查与脱落细胞学检查的病理对比分析

摘　　要：目的对比分析乳头溢液患者脱落细胞学检查、乳管镜检查后灌洗液液基细胞学检查结果与病理活检结果。方法100例具有乳头溢液临床表现的患者,均进行脱落细胞学检查、乳管镜灌洗液液基细胞学检查及病理活检。观察患者的病理活检结果,以病理活检结果为金标准,比较脱落细胞学检查、乳管镜灌洗液液基细胞学检查的诊断准确率。结果100例患者中,乳腺浸润性癌1例,乳腺原位癌1例,乳腺非典型增生2例,乳腺导管内乳头状瘤70例,乳腺良性病变(包括纤维腺瘤、纤维囊性乳腺病及普通型增生)22例,乳腺导管扩张症4例。乳管镜灌洗液液基细胞学检查对导管内乳头状瘤的诊断准确率及总准确率分别为85.71%、81.00%,均高于脱落细胞学检查的42.86%、43.00%,差异具有统计学意义(P<0.05);两组检查方法对乳腺浸润性癌、乳腺原位癌、乳腺非典型增生、乳腺良性病变及乳腺导管扩张症的诊断准确率比较差异均无统计学意义(P>0.05)。结论乳头溢液应用乳管镜灌洗液液基细胞学检查的临床应用价值较高,可显著提升诊断准确率,降低漏诊率。Objective To compare and analyze the results of exfoliated cytology,liquid-based cytology of mammary ductoscopy lavage fluid and pathological biopsy of patients with nipple discharge.Methods 100 patients with clinical manifestations of nipple discharge were all performed exfoliative cytology,lavage fluid liquid-based cytology after mammary ductoscopy and pathological biopsy.The pathological biopsy results were used as the gold standard to compare the diagnostic accuracy of exfoliative cytology and liqu id-b ased cytology of mammary ductoscopy lavage fluid.R esults Among the 100 patients,there were 1 case of breast invasive carcinom a,1 case of breast carcinom a in situ,2 cases of breast atypical hyperplasia,and 70 cases of breast intraductal papilloma,22 cases of benign breast lesions(including fibroadenoma,fibrocystic breast d isease and general hyperplasia),and 4 cases of breast ductal dilatation.The accuracy and total accuracy of liquid-based cytology of mammary ductoscopy lavage fluid for intraductal papilloma were 85.71%and 81.00%,respectively,which were higher than the 42.86%and 43.00%of exfoliative cytology,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the diagnostic accuracy between the two inspection methods for breast invasive carcinoma,breast carcinoma in situ,breast atypical hypeqilasia,benign breast lesions,and breast ductal dilatation(P>0.05).Conclusion Liqu id-based cytology of mammary ductoscopy lavage fluid shows high clinical value on patients with nipple discharge,and it can significantly improve the diagnostic accuracy and reduce the m issed diagnosis.
