摘 要:目的探究多模态MRI联合钼靶X线鉴别诊断乳腺良恶性肿瘤的价值。方法回顾性分析2018年9月至2019年9月在新乡市中心医院检查的200例乳腺占位性病变患者的多模态MRI及钼靶检查资料,以病理检查结果为标准,比较多模态MRI及钼靶检查结果与病理检查结果的一致性;采用ROC曲线分析多模态MRI、钼靶检查及联合检查对乳腺占位性病变的鉴别价值,比较单独检查、联合检查诊断乳腺占位性病变的灵敏度、特异度、准确度及对良恶性钙化点的检出率。结果多模态MRI检查结果与病理检查结果的一致性(Kappa=0.560)高于钼靶X线(Kappa=0.415);经ROC曲线分析,联合检测鉴别良恶性乳腺占位性病变的曲线下面积(AUC)为0.859,大于多模态MRI及钼靶X线单独检测的0.707、0.788;联合检测的灵敏度为94.52%,明显高于钼靶X线、多模态MRI的71.23%、78.08%,差异均有统计学意义(P<0.05);联合检测的特异度、准确度分别与钼靶X线及多模态MRI比较,差异均无统计学意义(P>0.05);联合检测的恶性钙化点检出率为91.43%,明显高于多模态MRI的68.57%,差异有统计学意义(P<0.05),但联合检测的恶性钙化点检出率与钼靶X线的恶性钙化点检出率(80.00%)比较,差异无统计学意义(P>0.05);联合检测的良性钙化点检出率为95.00%,明显高于多模态MRI、钼靶X线的75.00%、80.00%,差异均有统计学意义(P<0.05)。结论多模态MRI联合钼靶X线对乳腺良恶性肿瘤的诊断灵敏度较高,且对乳腺良恶性肿瘤具有诊断价值。Objective To explore the value of multimodal MRI combined with molybdenum target X-ray in the differential diagnosis of benign and malignant breast tumors.Methods The examination data of multimodal MRI and molybdenum target from the 200 patients with breast space-occupying lesions examined in Xinxiang Central Hospital between September 2018 and September 2019 were retrospectively analyzed.Taking pathological examination results as the standard,the consistency between multimodal MRI,molybdenum target and pathological examination results was compared.The differential value of multimodal MRI,molybdenum target and their combination for breast space-occupying lesions was analyzed by ROC curves.The sensitivity,specificity and accuracy of single examination and combined examination for breast space-occupying lesions,as well as the detection rates of benign and malignant calcification were compared.Results The consistency between multimodal MRI and pathological examination results was higher than that between molybdenum target X-ray and pathological examination results(Kappa:0.560 vs 0.415).ROC curve analysis showed that area under the curve(AUC)of multimodal MRI combined with molybdenum target for identifying benign and malignant breast space-occupying lesions was greater than that of multimodal MRI and molybdenum target X-ray alone(0.859 vs 0.707,0.788),and sensitivity of their combination was significantly higher than that of molybdenum target X-ray and multimodal MRI alone(94.52%vs 71.23%,78.08%),P<0.05.There was no significant difference in specificity and accuracy between combined detection and molybdenum target X-ray,multimodal MRI(P>0.05).The detection rate of malignant calcification by combined detection was significantly higher than that by multimodal MRI(91.43%vs 68.57%).The difference in detection rate of malignant calcification between combined detection and molybdenum target X-ray(91.43%vs 68.57%)was not statistically significant(P>0.05).The detection rate of benign calcification by combined detection was s
