摘 要:目的探讨急性心肌梗死患者心功能与认知功能的相关性。方法选取104例急性心肌梗死患者,根据Killip分级将其分为心功能Ⅱ级组(n=71)和心功能Ⅲ级组(n=33)。收集和比较两组患者的一般资料、血液学指标、心功能指标和认知功能指标,并采用Pearson相关或Spearman相关分析心功能指标与认知功能指标的相关性。结果心功能Ⅲ级组的血清N末端B型利钠肽前体(NT-proBNP)、肌红蛋白、高敏肌钙蛋白、三酰甘油、尿素氮水平高于心功能Ⅱ级组(P<0.05)。心功能Ⅲ级组的左心室射血分数(LVEF)、蒙特利尔认知评价量表(MoCA)总分、简易智能状态检查量表(MMSE)总分低于心功能Ⅱ级组(P<0.05)。急性心肌梗死患者的LVEF与MoCA总分和MMSE总分呈正相关(P<0.05),NT-proBNP水平与MoCA总分和MMSE总分呈负相关(P<0.05)。结论急性心肌梗死患者心功能与认知功能具有相关性,心功能水平越差者认知功能受损越重。Objective To investigate the correlation between cardiac function and cognitive function in patients with acute myocardial infarction(AMI).Methods A total of 104 AMI patients were enrolled and divided into KillipⅡcardiac function group(n=71)and KillipⅢcardiac function group(n=33)according to the Killip classification.General data,hematological indices,cardiac function indices,and cognitive function indices were collected and compared between the two groups,and Pearson correlation or Spearman correlation analysis was applied to the correlation between cardiac function indices and cognitive function indices.Results The levels of serum N-terminal pro-B-type natriuretic peptide(NT-proBNP),myoglobin,high-sensitivity troponin,TG and urea nitrogen in the KillipⅢcardiac function group were higher than those in the KillipⅡcardiac function group(P<0.05).The KillipⅢcardiac function group exhibited decreased left ventricular ejection fraction(LVEF),lower total scores on the Montreal Cognitive Assessment(MoCA)or on the Mini-Mental State Examination(MMSE)as compared with the KillipⅡcardiac function group(P<0.05).In AMI patients,LVEF was positively related to total score on MoCA or MMSE(P<0.05),NT-proBNP level was negatively related to total score on MoCA or MMSE(P<0.05).Conclusion Cardiac function correlates with cognitive function in AMI patients,those with poorer cardiac function level experience severer impairment of cognitive function.
