针刀灭活肌筋膜触发点术联合星状神经节阻滞治疗颈性高血压的疗效观察

摘　　要：目的观察针刀灭活肌筋膜触发点术联合星状神经节阻滞治疗颈性高血压的临床效果。方法将88例颈性高血压患者随机分为研究组和对照组,每组44例。对照组患者采用星状神经节阻滞治疗,研究组患者采用针刀灭活肌筋膜触发点术联合星状神经节阻滞治疗,两组均治疗3个疗程。分别于治疗前后采用疼痛视觉模拟量表(VAS)评分、颈椎功能障碍指数量表(NDI)评分评价两组患者颈项部疼痛程度与颈椎功能,采用经颅多普勒血流分析仪检测两组患者椎-基底动脉血流速度,观察两组患者血压变化,并评定两组疗效。结果研究组治疗总有效率为93.18%(41/44),对照组为77.27%(34/44),研究组疗效优于对照组(P<0.05);治疗后,两组收缩压、舒张压、疼痛VAS评分、NDI评分均低于治疗前,两组左、右侧椎动脉、基底动脉血流速度快于治疗前,且研究组上述指标改善均优于对照组(均P<0.05)。结论相比于单纯星状神经节阻滞,采用针刀灭活肌筋膜触发点术联合星状神经节阻滞治疗颈性高血压疗效更好,更能有效地降低患者血压水平,恢复椎-基底动脉血流,缓解疼痛症状,改善患者颈椎功能。Objective To observe the clinical effect of acupotomy inactivating myofascial trigger points combined with stellate ganglion block(SGB)on cervical hypertension.Methods Eighty-eight patients with cervical hypertension were randomly divided into study group and control group,with 44 cases in each group.Patients in the control group received SGB therapy,whereas patients in the study group received acupotomy inactivating myofascial trigger points combined with SGB therapy,and both groups were treated for three courses.Before and after treatment,the neck pain and cervical vertebra function of patients in the two groups were evaluated by the Visual Analogue Scale(VAS)pain score and Cervical Disability Index(NDI)score.The transcranial Doppler blood flow analyzer was employed to detect vertebrobasilar artery blood flow velocity in the two groups,changes in blood pressure were observed in the two groups,and the efficacy of the two groups was assessed.Results The total effective rate was 93.18%(41/44)in the study group and 77.27%(34/44)in the control group,the study group had superior efficacy than the control group(P<0.05);after treatment,SBP,DBP,VAS pain score and NDI score were lower and the blood flow velocities of left/right vertebral artery and basilar artery were faster in the two groups than those before treatment,and the study group was superior to the control group in the improvements of the indices mentioned above(all P<0.05).Conclusion Compared with simple SGB,acupotomy inactivating myofascial trigger points combined with SGB therapy exerts better efficacy for treating cervical hypertension,for it can reduce patients′blood pressure level more effectively,restore vertebrobasilar artery blood flow,ameliorate pain symptom and improve cervical vertebra function of the patients.
