侧脑室外引流尿激酶灌洗结合腰大池引流治疗脑室出血的临床研究

摘　　要：目的探究侧脑室外引流尿激酶灌洗结合腰大池引流治疗脑室出血的临床效果。方法 110例脑室出血患者,根据治疗方案不同分为对照组和观察组,各55例。对照组采取侧脑室外引流尿激酶灌洗治疗,观察组采取侧脑室外引流尿激酶灌洗结合腰大池引流治疗。对比两组脑室血肿消除时间、神志恢复时间、住院时间及并发症发生情况。结果观察组脑室血肿消除时间(5.15±1.15)d、神志恢复时间(4.20±1.30)d、住院时间(7.65±3.15)d均短于对照组的(8.25±2.35)、(7.65±2.65)、(13.51±2.36)d,差异具有统计学意义(P<0.05)。观察组并发症发生率为5.45%,低于对照组18.18%,差异具有统计学意义(P<0.05)。结论侧脑室外引流尿激酶灌洗结合腰大池引流治疗脑室出血的临床效果显著,可以加快脑室血肿消除速度,缩短患者神志恢复时间和住院时间,降低并发症发生率,应该在临床上进行推广应用。Objective To investigate the clinical effect of lateral ventricular external drainage and urokinase lavage combined with lumbar cistern drainage in the treatment of intraventricular hemorrhage. Methods A total of 110 patients with intraventricular hemorrhage were divided into control group and observation group according to different treatment methods, with 55 cases in each group. The control group was treated with lateral ventricular external drainage and urokinase lavage, and the observation group was treated with lateral ventricular external drainage and urokinase lavage combined with lumber cistern drainage. The elimination time of ventricular hematoma, mental recovery time, hospitalization time and occurrence of complications were compared between the two groups. Results The elimination time of ventricular hematoma(5.15±1.15) d, mental recovery time(4.20±1.30) d and hospitalization time(7.65±3.15) d of the observation group were shorter than(8.25±2.35),(7.65±2.65) and(13.51±2.36) d of the control group, and the difference was statistically significant(P<0.05). The incidence of complications of the observation group was 5.45%, which was lower than 18.18% of the control group, and the difference was statistically significant(P<0.05). Conclusion Combination of lateral ventricular external drainage, urokinase lavage and lumbar cistern drainage shows remarkable clinical effect on intraventricular hemorrhage, which can speed up the elimination of ventricular hematoma, shorten the mental recovery time and hospitalization time, and lower the incidence of complications. It should be widely promoted and applied in clinic.
