彩色多普勒超声对高血压左室肥厚伴心力衰竭的诊断价值探讨

摘　　要：目的探讨彩色多普勒超声诊断高血压左室肥厚伴心力衰竭的临床价值。方法选取90例高血压左室肥厚伴心力衰竭患者作为试验组,另选取90例健康体检者作为对照组。两组均使用彩色多普勒超声进行诊断。对比两组心脏彩超指标[两组左室舒张末期内径(LVEDd)、左房内径(LAD)、左室射血分数(LVEF)、舒张早期二尖瓣口最大血流速度与二尖瓣环舒张早期最大运动速度的比值(E/Ea)]水平及试验组不同心功能分级患者LVEDd、LAD、LVEF、E/Ea水平。结果试验组患者LVEDd(53.73±7.31)mm、LAD(49.88±1.48)mm、E/Ea(12.40±1.61)均大于对照组的(43.45±4.28)mm、(30.17±1.34)mm、(6.02±0.02), LVEF(55.93±3.54)%低于对照组的(69.13±1.09)%,差异有统计学意义(P<0.05)。心功能Ⅱ、Ⅲ级患者LVEDd、LAD及E/Ea均大于Ⅰ级, LVEF低于Ⅰ级,差异有统计学意义(P<0.05);Ⅲ级患者LVEDd、LAD及E/Ea均大于Ⅱ级, LVEF低于Ⅱ级,差异有统计学意义(P<0.05)。结论彩色多普勒超声检查可清晰显示高血压左室肥厚伴心力衰竭患者的心脏结构,通过对LVEDd、LAD、LVEF、E/Ea进行动态监测,进而了解患者的心功能,对病情的诊断具有重要价值。Objective To discuss the clinical value of color Doppler ultrasound in hypertensive left ventricular hypertrophy with heart failure. Methods There were 90 cases of hypertensive left ventricular hypertrophy with heart failure selected as the experimental group, and another 90 healthy subjects as the control group. Both groups were diagnosed by color Doppler ultrasound. Comparison was made on cardiac color Doppler ultrasound index [left ventricular end-diastolic diameter(LVEDd), left atrial inner diameter(LAD), left ventricular ejection fraction(LVEF), ratio of early transmitral flow velocity to mitral annulus early diastolic velocity(E/Ea)] level between the two groups and of patients with different cardiac function classifications in the experimental group. Results The LVEDd(53.73±7.31) mm, LAD(49.88±1.48) mm and E/Ea(12.40±1.61) of the experimental group were greater than(43.45±4.28) mm,(30.17±1.34) mm and(6.02±0.02) of the control group, and LVEF(55.93±3.54)% was lower than(69.13±1.09)% of the control group, and the difference was statistically significant(P<0.05). The LVEDd, LAD and E/Ea of the patients with cardiac function of grade Ⅱand Ⅲ were all greater than those of grade Ⅰ patients, and the LVEF was lower than that of grade Ⅰ patients, and the difference was statistically significant(P<0.05);the LVEDd, LAD and E/Ea of grade Ⅲ patients were greater than those of grade Ⅱ patients, and LVEF was lower than that of grade Ⅱ patients, and the difference was statistically significant(P<0.05). Conclusion Color Doppler ultrasonography can clearly show the cardiac structure of patients with hypertensive left ventricular hypertrophy with heart failure. Through dynamic monitoring of LVEDd, LAD, LVEF, E/Ea, the patient’s heart function can be understood, which is of great value for the diagnosis of the disease.
