轻比重液在老年股骨颈骨折患者行单侧股骨头置换术中的应用及睡眠质量比较

摘　　要：目的:了解轻比重液在老年股骨颈骨折患者行单侧股骨头置换术中的应用效果。方法:选取2017年1月至2019年1月北京百子湾和美妇儿医院行单侧人工股骨头置换术患者100例作为研究对象,按照随机数字表法随机分为轻比重液组和重比重液组,每组50例。轻比重液组选用1%罗派卡因和注射用水各1.5 mL配成0.5%的罗派卡因2~3 mL作为腰麻药;重比重液组选用1%罗派卡因和10%葡萄糖注射液各1.5 mL配成0.5%的罗派卡因2~3 mL作为腰麻药。比较2组效果和睡眠质量。结果:轻比重液组感觉阻滞起效时间和运动阻滞的起效时间明显短于重比重液组,差异有统计学意义(P<0.05);轻比重液组发生低血压、恶心呕吐、呼吸抑制的情况明显低于重比重液组,差异有统计学意义(P<0.05)。结论:轻比重液在老年患者行单侧股骨头置换术中应用是安全有效的,对血流动力学影响小,值得临床推广应用。Objective:To understand the application effect of light specific gravity liquid in unilateral femoral head replacement in elderly patients with femoral neck fracture.Methods:A total of 100 patients underwent unilateral artificial femoral head replacement from January 2017 to January 2019 were randomly divided into light specific gravity liquid group and heavy specific gravity liquid group,50 cases in each group.Light specific gravity liquid group Choose 1% ropivacaine and 1.5 mL each of water for injection to make 0.5% ropivacaine 2~3 mL as spinal anesthetics;Heavy specific gravity liquid group Choose 1% ropivacaine and 10% glucose injection each 1.5 mL mixed with 0.5% ropivacaine 2~3 mL as spinal anesthetic,and sleep quality comparison.Results:The onset time of sensory block and the onset time of motor block in heavy specific gravity liquid groupas significantly shorter than that in heavy specific gravity liquid group,The difference was statistically significant(P<0.05).The occurrence of hypotension,nausea,vomiting,and respiratory depression in light specific gravity liquid group was significantly lower than that in heavy specific gravity liquid group,The difference was statistically significant(P<0.05).Conclusion:The light specific gravity fluid is safe and effective in unilateral femoral head replacement in elderly patients,and has little effect on hemodynamics.
