摘　　要：目的:了解中西部农村地区高血压和2型糖尿病患者健康相关生命质量现状及影响因素,为改善慢病患者生命质量提供理论依据。方法:采用多阶段分层随机抽样方法,对湖北省宜昌市当阳市、贵州省铜仁市思南县35岁及以上的高血压和2型糖尿病患者进行问卷调查。健康相关生命质量采用EQ-5D量表测量并使用中国患者积分体系转换为健康效用值,采用Tobit回归分析影响因素。结果:纳入分析1129例,当阳、思南慢病患者健康效用值分别为0.891和0.704。年龄、就业状况、与医务人员沟通情况、体育锻炼情况、两周患病情况等均是影响两地慢病患者生命质量的重要因素。结论:中西部农村地区慢病患者健康状况不容乐观,应改善生活方式,加强对公民的健康教育,进一步提高就医可及性。Objective:To understand the status quo and influencing factors of the health-related quality of life(HRQoL)among patients diagnosed with hypertension and type 2 diabetes in central and western rural areas of China,and to provide a theoretical basis of improving the HRQoL of the patients who suffer from chronic disease.Methods:Using multi-stage stratified random sampling,we conducted a household survey among aged 35 and over patients with hypertension or type 2 diabetes in Dangyang county and Yichang city in Hubei province,Sinan county and Tongren city in Guizhou province,respectively.HRQoL was measured by EQ-5D scale and converted into health utility value by Chinese value set,and Tobit regression was used to explore the influencing factors of health utility.Results:A total of 1129 valid questionnaires were obtained.The health utility values of the patients in Dangyang and Sinan were 0.891 and 0.704,respectively.Age,employment status,communication with medical staff,situation of physical exercise and two-week morbidity were all crucial factors of the HRQoL.Conclusion:The issues of health status of patients of chronic diseases in the central and western rural areas of China is not very optimistic,and their lifestyle should be improved,health education for citizens should be strengthened,and access to medical treatment should be further improved.
