摘　　要：目的:探讨改良手动血栓清除术联合导管直接溶栓术治疗急性髂股静脉血栓临床疗效。方法:52例急性髂股静脉血栓患者,随机分为两组,对照组(n=26)、观察组(n=26)分别采用导管直接溶栓术(CDT)治疗、改良手动血栓清除术(MAT)联合CDT治疗,对比疗效差异。结果:与对照组相比,观察组血栓清除成功率、总有效率均较高(均P<0.05);观察组失血量、24 h患肢消肿程度高于对照组,CDT时间、住院时间短于对照组,尿激酶使用量少于对照组(均P<0.05);治疗后两组凝血指标、血栓后综合征(PTS)发生率两组比较均有统计学差异(均P<0.05);两组并发症发生率对比无统计学差异(P>0.05)。结论:改良MAT联合CDT治疗急性髂股静脉血栓的有效性及安全性高。Objective:To explore the curative effect of modified manual thrombectomy(MAT)combined with catheter-directed thrombolysis(CDT)in the treatment of acute iliofemoral vein thrombosis.Methods:52 patients with acute iliofemoral vein thrombosis were randomly divided into two groups.The control group(26 cases)was treated with CDT,and the observation group(26 cases)was treated with MAT and CDT.The difference in efficacy was compared between the two groups.Results:Compared with the control group,the observation group had higher thrombus clearance success rate and total effective rate(all P<0.05).The blood loss and the degree of detumescence of the affected limb in 24 hours were higher of the observation group than those of the control group,and CDT time and hospital stay were shorter than those of the control group,and the usage of urokinase was less than that of the control group(all P<0.05).After treatment,there were statistical differences in the coagulation indexes and the incidence of post-thrombotic syndrome(PTS)between the two groups(all P<0.05).There was no statistical difference in the incidence of complications between the two groups(P>0.05).Conclusion:Modified MAT combined with CDT is effective and safe in the treatment of acute iliofemoral vein thrombosis.
